Median nerve compression secondary to carpal osteophyte is extremely rare. We report a case, of carpal tunnel syndrome due to osteophytic bony projection in the carpal tunnel in a 54-year-old man with antecedent traumatic wrist injury before 20 years. To our knowledge, this is the second reported case.
