İzole travmatik serratus anterior paralizisinde endobutton sistemi ile skapulotorasik tespit yöntemi

Abstract

Kanat skapula (KS), omuzun diğer hastalıklarına göre daha az sıklıkla karşımıza çıkmaktadır. Skapulotorasik fiksasyonun ağrının azalmasında, kozmetik şikayetlerin düzelmesinde ve fonksiyonel iyileşmenin sağlanmasında etkili olduğu gösterilmiştir. En sık neden serratus anterior kası paralizisidir. Fakat başka birçok nedene bağlı olabi lir. 16 yaşında erkek hastada serratus anterior kası pa ralizisine (uzun torasik sinir) bağlı gelişen kanat skapula mevcut idi. Bu nedenle, şikayetleri olan hastaya skapulo torasik fiksasyon uygulandı. Stabilizasyon amacıyla daha önce uygulanmamış bir yöntem olan endobutton (Smith &Nephew) ile tesbit yöntemi kullanıldı. Hastanın 6 aylık izlenimi sonrası bir problem olmadan omuz hareketlerinin preoperatif döneme göre arttığı ve şikayetlerinin geçtiği gözlendi.

Scapular winging is a relatively rare disorder than the other shoulder diseases. Scapulothoracic fixation has been shown to be effective in relieving pain, improving cosmet ics, and restoring function. The most common cause is paralysis of the serratus anterior muscle. Paralysis of the serratus anterior muscle may result from a variety of causes. Sixteen year old male patient developed winging of the scapula after long thoracic nerve injury (serratus an terior muscle paralysis) following a blunt trauma. Scapu lothoracic fixation was considered in order to relieve the patient’s complaints. Stabilization of the scapulothoracic joint was performed using an undescribed method with the use of a endobutton system (Smith &Nephew). After a six-month follow up the patient’s complaint of pain dis appeared, shoulder imbalance was restored with an im provement in shoulder abduction and in flexion.
