Öğe A basic algorithmic surgical approach for nicolau syndrome(Wolters Kluwer Medknow Publications, 2020) Yeniocak, Ali; Özkan, Mustafa; Temel, Metin; Güneren, Ethem; Kelahmetoğlu, OsmanBackground: Nicolau syndrome (NS) is a rare complication that develops after the administration of intramuscular diclofenac sodium. The etiology and surgical treatments of 11 patients with NS were evaluated and studies in the literature were examined. The aim of this study was to compose a basic algorithm for surgical approaches to treat NS. Materials and Methods: Eleven patients were evaluated for NS between December 2013 and January 2018. Two patients did not accept treatment, and nine patients underwent surgical debridement of necrotic tissues. The tissue defects of five patients were closed with a fasciocutaneous flap and, in four patients, the defects were repaired primarily. Results: No complications, such as wound infection, wound dehiscence, seroma, or flap necrosis, were encountered. Of the seven patients who received concurrent antibiotic therapy, no patient had any problems at their follow-up (2-30 months). The results were satisfactory from an aesthetic and functional point of view. Conclusion: NS was more frequent in women with a high body mass index and high fat in gluteal regions. We considered that any kind of medication could lead to NS. Different methods are discussed for treatment. © 2020 Journal of Cutaneous and Aesthetic Surgery | Published by Wolters Kluwer - Medknow 154Öğe Incidence, etiology, and patterns of maxillofacial traumas in Syrian patients in Hatay, Turkey: A 3 year retrospective study(Ulusal Travma ve Acil Cerrahi Derneği, 2019) Arlı, Cengiz; Özkan, Mustafa; Karakuş, AliBACKGROUND: This study aimed to assess the demographics, clinical features, and treatment costs of maxillofacial trauma cases referred to our hospital during the Syrian civil war. METHODS: The study included 80 cases of maxillofacial trauma. Patients with additional pathologies were excluded from the study. The patients were examined with respect to their demographics and clinical characteristics, Injury Severity Score (ISS), Glasgow Coma Score (GCS), treatments applied, costs, and outcomes. RESULTS: A total of 80 patients included 76 (95%) males and 4 (5%) females, with the mean age of 29.05±9.97 years (range, 13–56 years), and 72 (90%) of them were Syrians injured in the war. The most common mechanism of trauma was the firearms injury in 72 (90%) cases, and the most frequently seen lesion was the mandible fracture (n=48, 60%). The ISS of all the patients was <16, as a severe trauma score. The GCS value was 8–12 in 4 (5%) patients, and 15 in 76 (95%). The most common treatment applied were the reduction and fixation, and graft-flap following fracture (n=12, 15%). The mean duration of hospitalization was 15.27±13.0 days (range, 2–60 days). All patients were discharged from hospital. The mean cost per case was calculated as 5,581.55±56.3 Turkish Lira (range, 772–18,697 TL) or 1,251.24±14.2 US Dollars (US$173–4,192). There was a significant correlation between the costs and the length of hospitalization (p<0.001, r=+0.729) and trauma scores (p=0.004, r=−0.616). CONCLUSION: Firearms-injured young males with mandible fractures were the most common group of maxillofacial trauma cases seen during the Syrian war. The intensity of patients and the cost of the hospital stay have significantly increased because of the ongoing conflict in the neighboring country of Syria.Öğe Team Work on burn and surgeon as a team leader(Gulhane Military Medical Academy, 2013) Karagöz, Hüseyin; Bayram, Yalçin; Kapi, Emin; Özkan, MustafaEnuresis is the most common psychosocial problems in childhood and affects the child's self-esteem and quality of life. According to International Children's Continence Society (ICCS), enuresis is defined as after a minimum of five years, involuntary or purposeful, repetitive, at least three consecutive months of day and /or night as urinary incontinence. Enuresis can occur for many reasons; biological, psychological, social. Aim of treatment child with enuresis solve the problem without disturbing the children's self esteem. Family takes the most important role at treatment. Nurses responsible the needs of children with enuresis and their families at every stage of health care services. Therefore, nurses know the causes, type and severity of enuresis, understand the effects of child and family life, know the treatment methods and should provide counseling services to families.Öğe An unusual Wohlfahrtia magnifica myiasis case localized in cutaneous and subcutaneous tissues in a patient with head-neck cancer(2014) Çevik, Cengiz; Kaya, Özlem Aycan; Akbay, Ercan; Özkan, Mustafa; Kahraman, Ahmet; Uçak, MuratAurikula tümörleri tüm baş-boyun tümörlerinin %6'sını oluşturur. Aurikula malign tümörleri genellikle skuamöz hücreli karsinom ve bazal hücreli karsinomdur. Myiasis sağlıklı insanlarda nadiren görülür. Genellikle mental retardelerde, psikiyatrik bozukluğu olanlarda, yaşlılarda, kişisel hijien ve özbakımı kötü kişilerde ve immune sistemi bozukluğu olanlarda görülebilen bir parazittir. Myiasis insanlarda özellikle topical ve subtopikal bölgelerde görülür fakat diğer bölgelerde nadiren görülmektedir. Literatürde ülkemizden bildirilen myiasis olgusu çok azdır. Baş boyun bölgesinde ve özellikle de kanser yarasında çok daha nadir görülmektedir. Bu çalışmada bilgimize göre literatürde sunulmamış olan baş boyun kanseri zemininde gelişen ve deri ve deri altı dokuları tutan myiasis (Wohlfahrtia magnifica) olgusunu sunmayı amaçladık.Öğe Yanıkta ekip çalışması ve ekip lideri olarak cerrah(2013) Karagöz, Hüseyin; Bayram, Yalçın; Kapı, Emin; Özkan, MustafaYanık travmaları organizmanın tüm işlevlerinde olumsuz değişikliklere yol açan bir travma türüdür. Yanık travmalarının tedavisinde multidisipliner yaklaşımların önemi büyüktür. Yanık tedavisinde yer alan disiplinler arasında cerrah, enfeksiyon uzmanı, rehabilitasyon uzmanı, psikiyatrist, hemşire gibi birçok branştan eleman yer almaktadır. Tedavinin organizasyonu aşamasında da disiplinlerin koordinasyonunu sağlayacak sorumlu tek bir uzmana ihtiyaç vardır. Bu görevi üstlenen kişiler sıklıkla yanık konusunda uzman cerrahlardan oluşmaktadır. Ülkemizde yanık tedavisiyle uğraşan yardımcı elemanların güncel bilgiler konusunda eğitim ihtiyacının bulunduğu, yapılan çalışmalarda gösterilmiştir. Yanık tedavisinin güncel gelişmeler ışığında yetkinliğini sürdürebilmesi için sorumlu cerrahın araştırma ve eğitim faaliyetlerine aktif olarak katılması ve çalışanlarına da bilgi aktarımı yoluyla eğitim vermesi, yanık merkezlerinin teknik ve beceriye dayalı pratiklerini daha ileriye taşıyacak temel yol olacaktır..
