Öğe Assessment of Gastroesophageal Reflux Disease in Chronic Obstructive Pulmonary Disease: Is Measuring Cardioesophageal Angle on Multidetector Computed Tomography Images Useful?(Bentham Science Publ Ltd, 2016) Unlu, Ebru; Ulasli, Sevinc S.; Kacar, Emre; Acay, Mehtap B.; Ozdinc, Serife; Atci, Nesrin; Balcik, CinarWe aimed to investigate whether there were any differences between patients with chronic obstructive pulmonary disease (COPD) and controls according to their cardioesophageal angles (COA) by Computed tomography (CT) and also the relationship between the degrees of COA and pulmonary function values in COPD patients. Degrees of COA were measured on curved oblique coronal CT images of 198 COPD patients with gastroesophageal reflux (GER) and 298 control subjects. A comparative analysis of COA values in two groups was performed and possible correlations between degrees of COA and pulmonary function parameters were evaluated. COA values were significantly higher in COPD patients compared to controls (p<0.001). The degrees of COA was negatively correlated with FVC and FEV1 values (% and L) in COPD patients (p< 0.001, r=-0.579; p<0.001, r=-0.64 and p<0.001, r=-0.65; p<0.001, r=-0.615, respectively). A positive correlation between age and COA was found in all the study population (p=0.002; r=0.134). A negative correlation between COA and BMI was found in COPD patients (p=0.037, r=-0.149). In conclusion, COPD patients have significantly increased COA value which is a predisposing factor for GER. Also, the association between obtuse COA and decreased levels of pulmonary function parameters in COPD patients may indicate reflux-induced bronchoconstriction and should be considered in COPD management as a non invasive indicator of reflux.Öğe Association of breast arterial calcification and osteoporosis in Turkish women(Professional Medical Publications, 2015) Atci, Nesrin; Elverici, Eda; Kurt, Raziye Keskin; Ozen, Derya; Unlu, Ebru; Araz, LeventObjective: Breast arterial calcification (BAC), medial calcific sclerosis of small to medium-sized muscular arteries, is a benign finding of mammographic evaluation. Previous studies have shown the relationships between BAC and systemic disorders such as cardiovascular disease, diabetes mellitus and hypertension. The aim of this study was to determine the association between reduced bone mineral density and BAC. Methods: The study population consisted of 567 women who had both mammography and bone mineral density evaluation. BAC (+) and BAC (-) women were compared for age, body mass index, postmenopausal duration, number of deliveries, breastfeeding duration, DM, HT, lipid treatment, osteopenia, and osteoporosis. Results: BAC was seen in mammographic evaluation of 179 women and 388 subjects without BAC accepted as the control group. There was a statistically significant relationship between age, postmenopausal duration, number of deliveries, history of DM, HT, lipid treatment and BAC. While the prevalence of osteopenia was higher in control group (52.8%), the rate of osteoporosis (48.7%) was higher in group with BAC. Conclusion: There was statistically significant relationship between BAC and osteoporosis in postmenopausal women. Determination of BAC in routine screening mammography might be helpful in both identifying women with risk of cardiovascular disease and osteoporosis.Öğe Congenital abnormalities of the ribs: evaluation with multidetector computed tomography(Pakistan Medical Assoc, 2017) Davran, Ramazan; Bayarogullari, Hanifi; Atci, Nesrin; Kayali, Alperen; Ozturk, Fatma; Burakgazi, GuelenObjectives: To evaluate congenital abnormalities of ribs using multidetector computed tomography. Methods: The retrospective study was conducted at Mustafa Kemal University Research Hospital, Hatay, Turkey and comprised data of patients aged 1-45 years who attended the Radiology Department for computed tomography of the thorax between January 2010 and July 2013. Multiplanar reconstructions, maximum intensity projections, and 3-dimensional images were acquired to investigate numerical and structural abnormalities of the ribs of the patients who underwent multidetector computed tomography for a variety of indications. Results: The study comprised 650 patients. Of them, 231(35.5%) were female and 419(64.5%) male. The overall mean age was 20.9 +/- 5.1 years. However, data of 90(13.8%) patients was excluded from cervical rib evaluation and of 120(18.5%) from lumbar rib evaluation as these regions were out of the section because of the positioning. Finally, 560(86.5%) patients were included in the cervical rib evaluations, and 530(81.5%) in the lumbar rib evaluations. All the 650(100%) patients were included in the thoracic rib evaluations. Numerical abnormalities were observed in cervical ribs in 19(3.6%), in thoracic ribs in 1(0.15%) and in lumbar ribs in 7(1.3%) cases. The structural abnormalities were bifid rib in 44(6.7%) and fused type in 17(2.6%) cases. Conclusion: Multidetector computed tomography enabled evaluation of the thoracic cage as a whole.Öğe Ductal Adenocarcinoma: A Rare Entity of Prostate Gland in a Chronic Lymphocytic Leukemia/Small Lymphocytic Lymphoma Patient(Derman Medical Publ, 2015) Ozgur, Tumay; Rifaioglu, Murat Mehmet; Sumbul, Ahmet Taner; Aydogan, Fusun; Atci, NesrinProstate cancer is the most common malignancy in men and ductal adenocarcinoma is a pathologic subtype with specific histological and clinical features. Seventy-six year-old male patient with chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL) admitted to our hospital with lower urinary tract symptoms. The last prostate specific antigen (PSA) level was 26 ng/ml and serial transrectal ultrasound guided biopsies were administered and benign prostate hyperplasia and non-specific prostatitis were the results of pathology reports. Due to the persistence of the symptoms transurethral resection of the prostate was performed. In the pathologic evaluation of the material adenocarcinoma focuses without stroma has been observed between the hyperplasic prostate tissues. The tumor has been diagnosed as ductal adenocarcinoma with 4 + 4 Gleason pattern score. Bone scintigraphy was revealed activity uptake on lomber vertebral column due to metastasis. Computerized tomography was revealed previous bilateral inguinal and right iliac lymphadenopathy due to CLL/SLL. Total androgen deprivation therapy and bilateral orchiectomy was applied. After three mounts according to biochemical and imaging results, radiotherapy cure began. Ductal adenocarcinoma is a rare subtype of prostate carcinoma with clinical behavior from that seen in conventional adenocarcinoma. On the other hand it is worth to point out the occurence of this entity as second malignancy during follow-up of CLL/SLL.Öğe Effect of Sickle Cell Anemia on Pelvic Dimensions: A Magnetic(Kowsar Publ, 2017) Atci, Nesrin; Gozukara, Ilay; Ozturk, Fatma; Burakgazi, Gulen; Kurt, Raziye Keskin; Karazincir, SinemBackground: The cesarean delivery rate tends to be higher in women with sickle cell anemia (SCA) and sickle cell trait (SCT) for various reasons. However, pelvic contracture, which may lead to cesarean sections, has never before been studied in these groups. Objectives: The aim of this study was to investigate magnetic resonance (MR) pelvimetry findings of women with SCA and SCT. Patients and Methods: We studied 66 women who underwent MR pelvimetry between March and June 2015 at our center. Our prospective study included 21 women with SCA, 20 women with SCT, and 25 control women. Sagittal inlet, sagittal mid-pelvis, transverse inlet, transverse mid-pelvis and transverse outlet diameters were measured on MR images. The mean diameters were compared with each other and with the control group Results: Sagittal inlet and transverse inlet were not significantly different between the groups. Also, mid-pelvic diameters for the sagittal and transvers mid-pelvis were similar between the groups. Even though the transverse outlet seems to be lesser in the SCA group compared with other groups, it was not statistically significant. Conclusion: Our study demonstrated that the pelvic roofs of women with SCA and SCT are indistinguishable from healthy women. Thereby, maternal pelvic structures that impact delivery types may not be significantly changed in SCA and SCT diseases. We believe that when making decisions about modes of delivery, these results should be taken into consideration.Öğe Giant Retroperitoneal Liposarcoma with Multi-Organ Involvement(Derman Medical Publ, 2014) Ugur, Mustafa; Orlic, Cem; Akkucuk, Seckin; Koca, Yavuz Savas; Atci, NesrinRetroperitoneal liposarcomas are very rare tumors with poor prognosis. The diagnosis can be delayed since there are no specific symptoms and findings. Surgical resection is the primary treatment choice. Obtaining negative surgical margins should be the primary aim., Multiple organ resections may be essential in cases with huge liposarcoma and incomplete resection leads to local recurrences. There is still an ongoing controversy in the medical treatment of liposarcoma. In this study we presented a patient with liposarcoma who was advised for surgery due to retroperitoneal mass 2 years ago but refused so the mass grew to a huge size invading left kidney, left adrenal gland and left mesocolon. We aimed to point out organ invasion and huge masses of liposarcoma are not contraindications for resection, and resections of these huge tumors improve quality of life. All retroperitoneal masses should be aggressively excised and patients should be closely followed up.Öğe Increased Epicardial Fat Tissue Is a Marker of Subclinic Atherosclerosis in Ankylosing Spondylitis(Turkish League Against Rheumatism, 2014) Ustun, Nilgun; Kurt, Mustafa; Atci, Nesrin; Yagiz, Erman; Guler, Hayal; Turhanoglu, AyseObjectives: This study aims to assess subclinical atherosclerosis markers such as epicardial fat thickness, carotid intima media thickness (CIMT), and aortic stiffness index (ASI) in ankylosing spondylitis (AS) patients as well as to investigate the relationship between the subclinical atherosclerosis markers and disease activity/function/mobility indices. Patients and methods: Twenty-six AS patients (22 males, 4 females; mean age 43 years) and 26 age-and sex-matched healthy controls (21 males, 5 females; mean age 43 years) were included. Patients and controls with any reported cardiovascular disease or other comorbidities were excluded. Disease activity, functional capacity and spinal mobility were measured using the Bath Ankylosing Spondylitis Disease Activity Index, Bath Ankylosing Spondylitis Functional Index, and Bath Ankylosing Spondylitis Metrology Index, respectively. All patients underwent complete transthoracic echocardiographic examination including epicardial fat thickness and ASI, and sonographic examination including CIMT. Results: There were no significant differences in demographical and cardiovascular characteristics between AS patients and healthy controls (p>0.05). Epicardial fat thickness (5.15 +/- 1.13 vs. 4.11 +/- 1.22; p=0.003), CIMT (0.70 +/- 0.16 vs. 0.60 +/- 0.10; p=0.012) and ASI (14.2 +/- 10.8 vs. 8.6 +/- 3.1; p=0.018) were significantly increased in patients with AS compared to the healthy controls. There was no significant correlation between the subclinical atherosclerosis markers and disease activity/function/mobility indices (p>0.05). The hernia was repaired without any complications.Öğe Magnetic Resonance Imaging Findings of the Abducens Nerves in Type 1 Duane's Retraction Syndrome(Taylor & Francis Inc, 2014) Tuzcu, Esra Ayhan; Bayarogullari, Hanifi; Atci, Nesrin; Basarslan, Fatmagul; Coskun, Mesut; Yilmaz, Cahide; Ilhan, NiluferPurpose: To investigate nervus abducens and extraocular mucles in patients with Type 1 Duane's retraction syndrome using high-definition magnetic resonance imaging. Methods: The study included 10 patients with Type I Duane's retraction syndrome who underwent magnetic resonance imaging (MRI) of the brain and orbits. Results: Overall, 10 cases were included in the study. There were seven women and three men. The mean age was 5.2 years (1-15 years). MRI of the abducens nerve was performed in all cases. Of the cases, the left eye was involved in eight cases, whereas the right eye was involved in two cases. There was no bilateral eye involvement. Among the 10 patients clinically diagnosed as Type 1 Duane's retraction syndrome, the abducens nerve could not be visualized in eight cases, whereas the nerve was hypoplastic in one case and bilateral abducens nerves were present in one case by MRI. The extraocular muscles were normal in all cases on T2 weighted coronal MRI of the orbits. Conclusion: Absence of abducens nerve and normal extraocular muscles was detected in patients with Type 1 Duane's retraction syndrome at the affected side.Öğe Orbital compression syndrome complicated by epidural hematoma and wide cephalohematoma in a patient with sickle cell disease(Mosby-Elsevier, 2014) Ilhan, Nilufer; Acipayam, Can; Aydogan, Fusun; Atci, Nesrin; Ilhan, Ozgur; Coskun, Mesut; Daglioglu, Mutlu CihanOrbital wall infarctions resulting in orbital and epidural hematomas are rare manifestations of sickle cell disease (SCD). We report orbital compression syndrome associated with an epidural hematoma and wide cephalohematoma in a 15-year-old boy with SCD. An infarcted orbital bone was observed on magnetic resonance imaging and three-phase bone scintigraphy with Technetium-99m methylene diphosphonate. The patient recovered completely without surgical intervention at the end of the fourth week. Blunt traumas caused by automobile accidents are the prior reasons to this. Central venous catheterization, ballistic trauma, cystic medial necrosis, fibromuscular dysplasia, atherosclerosis, and other inflammatory processes are some other potential reasons for pseudoaneurysm. In contrast to true aneurysms, it does not contain 3-layer structure with adventitia, media, and intima sublayers in its wall. The wall contains coagulum and fibrous capsules. Clinically, after trauma, it presents itself as swelling after days to weeks of damage to vascular walls. After the introduction of antibiotics, complications have started to be observed more frequently as a result of chronic otitis media and especially in children. Because clinical findings changed and became indistinct, the diagnosis of otitis and mastoiditis has been made occasionally with imaging findings only after complications occurred. Multidetector computed tomography and magnetic resonance imaging are efficient and sufficient methods in the rapid diagnosis and should be immediately referred methods.Öğe Red blood cell supports in severe clinical conditions in sickle cell diseases(Medi+World Int, 2016) Helvaci, Mehmet Rami; Atci, Nesrin; Ayyildiz, Orhan; Muftuoglu, Orhan Ekrem; Pocock, LesleyBackground: Sickle cell diseases (SCDs) are accelerated atherosclerotic processes. We tried to understand whether or not there is a prolonged survival with the increased number of red blood cells (RBC) transfusion in the SCDs. Methods: As one of the significant endpoints of the SCDs, cases with chronic obstructive pulmonary disease (COPD) and without, were collected into the two groups. Results: The study included 428 patients (221 males). There were 71 patients (16.5%) with COPD. Mean age was significantly higher in the COPD group (32.8 versus 29.8 years, P=0.005). Male ratio was significantly higher in the COPD group, too (78.8% versus 46.2%, P<0.001). Smoking (35.2% versus 11.4%, P<0.001) and alcohol (7.0% versus 1.9%, P<0.01) were also higher among the COPD cases. Beside these, priapism (14.0% versus 3.0%, P<0.001), HCV RNA positivity (2.7% versus 0.5%, P<0.05), cirrhosis (8.4% versus 3.3%, P<0.05), leg ulcers (23.9% versus 12.0%, P<0.01), digital clubbing (25.3% versus 6.7%, P<0.001), coronary heart disease (23.9% versus 13.7%, P<0.05), chronic renal disease (15.4% versus 7.0%, P<0.01), stroke (16.9% versus 8.1%, P<0.01), and mean transfused RBC units in their lives (63.8 versus 33.0, P=0.003) were all higher among the COPD cases. This was probably due to the higher number of transfused RBC units; the mean age of mortality was also higher in the COPD group, significantly (38.3 versus 30.4 years, P=0.04). Conclusion: SCDs are chronic catastrophic processes on vascular endothelium terminating with accelerated atherosclerosis induced endorgan failures in early years of life. RBC supports in severe clinical conditions probably prolong survival of the patients.Öğe The role of MR Myelogram in the diagnosis of traumatic pseudomeningocele(2013) Bayaro?ullari, Hanifi; Atci, Nesrin; Altaş, Murat; Aras, Mustafa; Özden, RaifTraumatic pseudomeningocele is a valuable indicator of nerve root injury. Secondary to trauma, the traction forces firstly tear meninges and later if the traction forces are great enough, nerve root avulsions occur. In this article, we aimed to show pseudomeningoceles localized at the level of brachial and lumbar plexus secondary to nerve root injury. We retrospectively reviewed the patients who admitted to our hospital between 2009 and 2011 due to various accidents. After clinical and radiological examinations, spinal root injuries were detected in six patients at different levels. Brachial plexopathy was detected in four patients and lumbosacral plexopathy was detected in two patients. Pseudomeningocele is a valuable indicator of nerve root injury. It occurs after dural torn like pouch. MR and MR myelography is the best imaging modality to indicate its' pouch and cerebrospinal fluid (CSF) within it. MR and MR myelography are best effective imaging modalities to indicate the pseudomeningoceles due to nerve root injury.Öğe Utility of intravaginal ultrasound gel for local staging of cervical carcinoma on MRI(Elsevier Science Inc, 2016) Atci, Nesrin; Ozgur, Tumay; Ozturk, Fatma; Dolapcioglu, Kenan SerdarPurpose: The aim of this study is to evaluate the utility of intravaginal ultrasound gel for the staging of cervical carcinoma on magnetic resonance imaging (MRI) and to confirm the results with pathological staging. Methods: Nine patients were included in the study. T2-weighted images were compared without and with vaginal gel (VG). Results: Five patients were evaluated as overstaged on MRI without VG. After VG, the results were compatible with the pathological stages in all patients. Conclusions: We suggest that this technique is easy, well tolerated, and effective, and it increases the accuracy rate of MRI staging in early cervical cancer. (C) 2016 Elsevier Inc.
