    Angioedema is an unfavorable factor for the response to omalizumab in chronic spontaneous urticaria: A retrospective study

    Antihistamines are the first-line treatment option for chronic urticaria. In recent years, omalizumab, an anti-immunoglobulin-E humanized monoclonal antibody, has been used in patients with recalcitrant disease. The present study aimed to retrospectively evaluate the efficacy and safety of omalizumab and determine whether there was a difference between complete and partial responses to omalizumab with respect to age, gender, disease duration and coexistence of angioedema. From May 2014 to December 2016, a total of 40 refractory chronic urticaria patients were treated with omalizumab. Complete response was observed in 19 (47.5%) patients, and partial response was observed in 18 (45%) patients. There were no statistically significant differences between the rates of complete and partial responses in patients with respect to gender, age, and disease duration. However, complete response was more frequent (60%) in patients without angioedema. Remission was observed in 40.5% (n = 15) of patients, and the follow-up time was 5.5 +/- 2.4 months. There was a statistically significant association between remission and coexistence of angioedema (p < .05). Eighty-seven percent (13/15) of the remission patients did not have angioedema. Thus, omalizumab can be used effectively and safely in refractory chronic urticaria patients. However, the coexistence of angioedema may be an unfavorable factor for complete response and remission.

    Association of folic acid receptor ? in maternal serum with neural tube defects

    Objective: To evaluate whether serum folic receptor alpha levels are changed in women whose previous pregnancies were complicated with neural tube defects (NTDs). Methods: This was a case-control study that included 41 women as the control group who had previously had at least one healthy pregnancy and 37 women as the study group who had a previous pregnancy complicated with NTDs. Blood samples were obtained from all of the participants six weeks after the termination of pregnancy or delivery of a baby. Serum folate receptor alpha concentrations were analyzed using a commercially available enzyme-linked immunosorbent assay (ELISA) kit. Results: The mean concentrations of serum folate receptor alpha were significantly lower in the NTD cases compared to those in the control group (p = 0.02). There was no significant difference in mean serum folate titers between the NTD cases and the control group (p = 0.07). Conclusion: Low serum folic acid receptor alpha levels in the current study did not appear to be a regulatory marker of maternal folate homeostasis per se but rather a factor that contributed to the development of NTDs.

    The Co-Existence of Vulvar Lichen Sclerosus, Ulcerated Calcinosis Cutis, and Dermatomyositis: Coincidence or Immunological Mechanism?

    Calcinosis cutis is a condition characterized by the deposition of calcium salts in the skin and subcutaneous tissues, and patients suffering from it encounter various connective tissue disorders, such as dermatomyositis (DM), scleroderma, and systemic lupus erythematosus. Although calcinosis cutis is frequently accompanied by juvenile dermatomyositis, rare cases have been reported in adult patients with DM. On the other hand, lichen sclerosus (LS) is a chronic inflammatory disease of the skin and mucosal surfaces. In the present report, we present a rare case of a 71-year-old patient with DM accompanied by ulcerated calcinosis cutis and vulvar LS. (Ann Dermatol 23(S3) S375 similar to S379, 2011)

    Diagnostic accuracy of angiopoietin-like protein 2 and atherosclerotic variables in psoriasis vulgaris using ROC analysis

    Aim: Psoriasis vulgaris is a chronic inflammatory skin disease associated with various comorbidities such as metabolic syndrome and cardiovascular disease. The disease is affected by many variables. Angiopoietin-like protein 2 (ANGPTL2) is a proinflammatory cytokine, and increased serum levels are associated with inflammation, obesity, dyslipidemia, metabolic syndrome, and atherosclerosis. The aim of this study was to determine the diagnostic accuracy of the new variable ANGPTL2 and other atherosclerotic variables for psoriasis, using Receiver Operating Characteristic (ROC) analysis. Material and Methods: The study included 41 psoriasis patients and 43 healthy volunteers. Ultrasound examination of the carotids and carotid intima-media thickness (CIMT) measurements were performed in all participants. Serum ANGPTL2 levels, lipid levels/ratios and anthropometric measurements were recorded. A ROC curve of multivariate statistical methods was used for statistical evaluation. Results: According to the results of the ROC curves, seven diagnostic variables (ANGPTL2, BMI, weight, systolic blood pressure, waist circumference for females, uric acid and LDL-cholesterol) were found to be significant. The highest Area Under the Curve (AUC) value was found for ANGPTL2; AUC=0.721, 95% CI: 0.595-0.847, p=0.002. There was no significant difference in the lipid ratios and CIMT measurements between psoriasis patients and healthy controls. Discussion: This study revealed that increased ANGPTL2 levels in psoriasis patients are evidence of the strong relationship between the disease and atherosclerosis. Therefore, the measurement of serum ANGPTL2 level may be a new marker that can be used to determine the risk of early atherosclerosis and metabolic complications in psoriasis patients.

    DLQI scores in patients with keloids and hypertrophic scars: a prospective case control study

    P>Background: Keloids and hypertrophic scars (HTS) can cause functional impairment and psychosocial burdens, both of which affects quality of life (QoL). Our aim was to compare Dermatology Life Quality Index (DLQI) scores in patients with keloids and HTS to those of psoriasis patients and normal controls. Patients and Methods: Forty-eight consecutive patients with keloids and HTS and 48 with psoriasis vulgaris attending our dermatology outpatient clinic, as well as 48 sex- and age-matched healthy controls completed the DLQI. Results: Total DLQI scores of patients with keloids and HTS (7.79 +/- 5.10) and psoriasis (8.73 +/- 5.63) were comparable and significantly higher than that of healthy controls (0.58 +/- 0.77). No significant difference were found between patients with psoriasis and patients with keloids and HTS in terms of the total DLQI scores and the subscale scores (p > 0.05) except treatment (p < 0.05) sub-scale scores which were higher in psoriasis. Conclusions: The QoL of patients with keloids and HTS is impaired as much as that of those with psoriasis. The DLQI questionnaire is a reliable and valid instrument for assessing the QoL in patients with keloids and HTS.

    Frequency, Distribution and Genotyping of Malassezia Species in Patients with Psoriasis vulgaris

    Background: Malassezia species are reported to play a role in the etiology of Psoriasis vulgaris. Objective: The aim of this study was to determine the presence, frequency, distribution, and genotyping of skin colonization of Malassezia species in Psoriasis vulgaris and to compare with healthy individuals and to investigate its relationship with the severity of the disease. Methods: Skin samples were taken from scalp, arm, body, and leg of 34 psoriasis patients (lesional/ non-lesional skin) and 30 healthy volunteers. Overall, 392 skin scraping samples were taken for the isolation of Malassezia species, which were incubated on the modified-Dixon agar. Conventional culture methods were used for Malassezia species identification. In isolates, genotyping was carried out by PCR-RFLP method. Results: In the samples from psoriatic lesions, most frequently isolated Malassezia species were M.globosa and M.furfur. Similarly, the most frequently isolated species in healthy volunteers was M. globosa; followed by M.restricta and M.sympodialis. The M.furfur isolation rate in psoriatic scalp and leg lesions of the patients was significantly higher than in healthy volunteers. There was no relationship between the severity of the disease and the isolated species. Conclusion: It was found that there was a difference between patients with psoriasis and healthy controls regarding presence and frequency of Malassezia species. Therefore, our study results support the view that Malessezia species may be associated with the etiopathogenesis of psoriasis. In addition, we surmise that the treatment applications for the regulation of skin microbiota of psoriasis patients will contribute positively to the treatment of psoriasis.

    Impaired Pulmonary Function in Patients with Psoriasis

    Background: Psoriasis is associated with chronic obstructive pulmonary disease. There is no study on the spirometric pulmonary function testing in patients with psoriasis. Objective: The aim of this study was to compare the spirometric parameters in patients with psoriasis and controls. Methods: Ninety-six patients with psoriasis and 60 sex- and age-matched control subjects were included in this study. Spirometric pulmonary function testing, including percent forced vital capacity (FVC%), percent forced expiratory volume in the 1st second (FEV1%), forced expiratory flow at 25-75% of FVC (FEF25-75%), and FEV1/FVC ratio, was performed in all study subjects. Results: The mean FEV1/FVC ratio and FEF25-75% were significantly lower in the psoriasis patients than in the controls (82.4 +/- 6.3 vs. 90.7 +/- 10.7, p < 0.001, and 86.7 +/- 24.2 vs. 94.8 +/- 23.0, p = 0.04, respectively). Both FEV1/FVC ratio and FEF25-75% were significantly associated with the presence of psoriasis (p < 0.001 and p = 0.029, respectively). Conclusion: Psoriasis patients had lower mean FEV1/FVC ratios and FEF25-75%, compared with the control subjects. FEV1/FVC and FEF25-75% are independently associated with the presence of psoriasis.     Increased Amount of Visceral Fat in Patients with Psoriasis Contributes to Metabolic Syndrome

    Background: Psoriasis is associated with obesity and metabolic syndrome. Metabolic syndrome is associated with visceral fat accumulation. There is no study on the accumulation of visceral fat in patients with psoriasis. Objective: The aim of this study was to compare the visceral fat accumulation in patients with psoriasis and controls. Subjects and Methods: 46 patients with psoriasis and 46 sex-and age-matched control patients were included in this study. The abdominal fat area [visceral fat area (VFA), subcutaneous fat area (SFA) and total fat area (TFA)] at the level of the umbilicus was evaluated by computed tomography. Results: The mean VFA value and VFA/SFA ratio of the psoriasis patients were significantly higher compared with the control patients (123.4 +/- 80.3 vs. 81.2 +/- 59.8 cm(2) and 0.734 +/- 0.593 vs. 0.491 +/- 0.336; p = 0.005 and p = 0.017, respectively). Fasting blood sugar levels were also found to be significantly higher in psoriasis patients, compared with the control patients (101.8 +/- 43.5 vs. 83.4 +/- 9.1 mg/dl; p = 0.005). Multiple linear regression analysis indicated that waist-to-hip ratio, age, body weight, the presence of psoriasis and metabolic syndrome were significantly associated with VFA.     Induction of allergic contact dermatitis and pervasive face edema due to Brazilian keratin treatment

    Brazilian keratin treatment (BKT) is a prevalent hair straightening method widely used by women in the world. The degree of concentration of formaldehyde safe for sensitive patients is still obscure. Report claims that these products contain intolerably high levels of formaldehyde. Yet, hair straightening products may lead to severe allergic contact dermatitis, by means of the responsible allergens such as formaldehyde and its releasers. This case report presents the case of a 37-year-old female patient developing extensive edema of the face and acute inflammatory changes in the scalp from chemical-induced irritation, on the day following the application of BKT to straighten her hair.

    Lipoid proteinosis in the eastern Mediterranean region of Turkey

    Background: Lipoid proteinosis (LP), also known as hyalinosis cutis et mucosae or Urbach-Wiethe disease, is a rare autosomal recessive genodermatosis. Hyalin-like material is deposited in multiple organs, including the skin, oral mucosa, larynx, and brain. Only about 300 cases have been reported in the literature so far. Occurrence of LP in siblings is also rare. The reasons for relatively large number of cases, the clinical features of the patients, and the association of LP with other clinical conditions are described in this article. Aims: The aim of this study is to show that LP is not rare in Turkey and consanguineous marriage is still a social problem. Methods: We included patients between the years of 2008-2011 with lesions resembling LP. Based on the findings of clinical and histological examination of all cases, a diagnosis of LP was made. Results: We presented six different families with multiple family members suffered from LP. In total, we determined eight children and six adult patients. Three of eight children were from the same family (sisters), the other three children belonged to another family (brothers), two children were from another family (brother and sister), and the adult patients were from three different families. Conclusions: Patients with LP are likely to present first to a dermatologist because of the appearance of their skin; therefore, it is important that the dermatologic diagnosis is not to be missed. We described patients with LP and discuss the salient features of this disease.

    Narrow-band ultraviolet B phototherapy in childhood

    Background: Narrow-band ultraviolet B (UVB) has been used for skin diseases in children. However, the use of phototherapy in childhood has limited due to the possible risk of skin cancer. The aim of this study is to evaluate the efficacy and safety of the narrow-band UVB phototherapy in children for the treatment of various skin diseases. Methods: The data of the children aged 16 years or under who were treated with narrow-band UVB in our clinic's phototherapy unit were analyzed, retrospectively. Results: The study included 77 children (42 girls, 35 boys) aged 5-16 years (mean 12.18 +/- 3.53) who were treated with a total of 81 courses of narrow-band UVB phototherapy in the last 4.5 years. The majority of patients had vitiligo (47%) and psoriasis (39%). Thirty-four (44.2%) patients showed complete response, 15 (19.5%) good response, 12 (15.6%) partial response, 5 (6.5%) poor response and 11(14.3%) no response. Acute side effects of phototherapy were observed only in 11 (14.3%) patients. Conclusions: Narrow-band UVB phototherapy is an effective and well-tolerated treatment in childhood.

    Punch Biopsy Results in Misdiagnosis of Pilomatrixoma

    Punch biopsy results in misdiagnosis of clinically unsuspected giant pilomatrixoma located over the parotid gland. This study presents a case of pilomatrixoma that was misdiagnosed as a malignant epithelial tumor using punch biopsy. A 25-year-old male was admitted to our clinic for the evaluation of a mass measuring 7x8x8 cm located over the parotid gland. The patient had previously undergone punch biopsy at another clinic because of a lesion in the parotid gland. Punch biopsy revealed a malignant epithelial tumor. The patient underwent excisional biopsy at our clinic. After the biopsy, the residual skin defect was treated using full-thickness skin grafts. The facial nerve and parotid gland were preserved during the biopsy. Histopathological examination of the excisional biopsy material revealed pilomatrixoma. Punch biopsy may result in misdiagnosis of skin lesions in the parotid gland. A differential diagnosis for benign tumors such as pilomatrixoma is essential prior to an aggressive surgical intervention of the parotid gland.

    Relationship between Helicobacter Pylori and Parapsoriasis

    Aim: Parapsoriasis is a rare skin disorder characterized by erythematous, squamous, atrophic patchy lesions. Its etiopathogenesis and trigger factors still remain unclear. Parapsoriasis cases carry an increased risk of transformation into cutaneous T cell lymphoma. In the literature, there is no data about Helicobacter pylori ( H. pylori) frequency in parapsoriasis. This study aims to evaluate the relationship between H. pylori infection and parapsoriasis. Materials and Methods: The study included thirty patients with parapsoriasis who had no dispeptic complaint and thirty healthy individuals. The patient group was divided into two groups as patients with small plaque parapsoriasis (SPP) and those with large plaque parapsoriasis (LPP) based on clinical examination. Carbon 14 urea breath test was performed to evaluate presence of H. pylori in both patient and control groups. Results: Parapsoriasis cases had a significantly higher rate (18/30, 60.0%) of H. pylori positivity than controls (9/30, 30.0%) (p=0.020). Whereas patients with SPP and patients with LPP had similar rates of H. pylori positivity (55.6% vs 44.4%, p=0.279). Comparison of H. pylori status according to gender it was found that majority of H. pylori positive individuals were men, indicating a significant difference (p=0.014). Conclusion: Our results revealed that H. pylori infection is increased in patients with parapsoriasis. Therefore, it was thought that H. pylori may be associated with parapsoriasis and this agent may play a role in the etiopathogenesis of the disease

    Vitiligo Coexistent with Psoriasis: Four Case Reports and Review of the Literature

    Vitiligo and psoriasis are both common disorders. However, psoriasis and vitiligo occuring contemporaneously in the same patient is rare. The etiology of the coincidence remains uncertain, although possibility of a Koebner-like phenomenon has been proposed. Herein we report four cases of vitiligo coexistent with psoriasis and review of the literature.
