Öğe Acute Mesenteric Ischemia: a Disease Still Challenging Surgeons(Springer India, 2022) Beyaz, Metin Onur; Demir, Ibrahim; Omeroglu, Sinan; Ata, Emin CanAcute mesenteric ischemia is a medical emergency that has a high incidence of morbidity and mortality. The most effective way to reduce its consequences is to establish early diagnosis and revascularization. Here, we aimed to present our management and outcome. In this case series analysis, patients with acute mesenteric ischemia were examined from January 2013 to September 2018. Thromboembolectomy was applied by using a 2-3 French sized Fogarty catheter until the superior mesenteric artery flow was restored. Endarterectomy and saphenous vein patchplasty were preferred in atherosclerotic arteries. Nineteen patients were examined. Hypertension (63.2%) and atrial fibrillation (57.9%) were the most common seen comorbid factors. After embolectomy, arterial pulsation came back in 17 (89.5%) patients. Two patients had saphenous vein bypass due to the severe atherosclerosis of the mesenteric artery. Seven (36.8%) patients died on early postoperative period. Multiorgan failure developed in 2 (10.5%) patients. The survival rate was 42.1% in 46 SD17.3 months follow-up. Acute mesenteric ischemia has high mortality and morbidity because of the local and systemic effects of comorbid diseases and intestinal ischemia. The main goals of treatment should be maintaining body auto-regulation, establishing revascularization of the ischemic intestine and removing necrotic tissues.Öğe Carotid artery screening in high risk asymptomatic individuals: outcomes of 3000 carotid screening(Mexican Acad Surgery, 2022) Onur-Beyaz, Metin; Demir, Ibrahim; Can-Ata, EminObjectives: The primary aims of the study to evaluate the efficacy of carotid screening tests to identify asymptomatic carotid artery stenosis among high-risk patients and early prevention of stroke. Background: The estimated prevalence of asymptomatic severe carotid stenosis (>_70%) in the general adult population ranges up to 3.1%. However, the prevalence is higher in comorbid individuals. This makes it important to perform screening testing for early diagnosis and treatment in predetermined high-risk patients. Material and methods: In this prospective study, 3000 selected patients screened during March 2017 and September 2018 at the most populated family health center. Participants selected among registered individuals who have at least one of the risk factors such as hypertension, hyperlipidemia, diabetes mellitus, obesity, and smoking. All the participants were asymptomatic and above the age of 55. Bilateral carotid artery screening performed by Duplex Ultrasonography (DUSG) at the first call and one year later. Patients with severe carotid and/or coronary artery stenosis treated by surgical revascularization or stent implantation in the light of the latest guidelines. Conclusion: Carotid screening among high-risk asymptomatic individuals is of great importance to identify severe carotid artery as well as coronary artery stenosis. Patient education during screening may play a crucial role in preventing the disease.Öğe Comparison of atherectomy, drug-eluting balloon, and combined treatment efficiency by near infrared spectroscopy(Mexican Acad Surgery, 2021) Onur-Beyaz, Metin; Demir, Ibrahim; Ozer-Ulukan, M.Atherosclerosis is a systemic pathology involving the entire vascular system. Smoking, hyperlipidemia, diabetes mellitus, advanced age, and hypertension are known risk factors for the development of atherosclerosis. Surgical treatment in peripheral arterial disease (PAD) has been replaced by endovascular treatment options in recent years due to mortality and morbidity. In our study, we aimed to present the effectiveness of atherectomy, drug-eluting balloon, and combined therapy with values determined by near-infrared spectroscopy (NOS) in patients they have atherosclerotic PAD. Materials and methods: Patients with intermittent claudication or critical leg ischemia (Rutherford class 3 or more) have been viewed in the study. There were 30 male and 9 female patients. Patients were followed up before, at the 1st h after and at the 3rd month after the procedure, with simultaneous ankle-brachial pressure index (ABPI), walking distance measurements, and measurement of tissue oxygenation by NIRS method. Results: When patients with similar post-operative ABPI measurements were evaluated with MRS, venous oxygenation was observed higher in patients used treatment methods combined with atherectomy. According to these results, we thought that treatment methods combined with atherectomy have increased microcirculation and collateral circulation more than other methods.Öğe Effect of Priming on Germination of Lagenaria siceraria Genotypes at Low Temperatures(Ankara Univ, Fac Agr, 2007) Kenanoglu, Burcu Begum; Demir, Ibrahim; Mavi, Kazim; Yetisir, Halit; Keles, DavutThis work was carried out to test whether germination of fifteen Lagenaria siceraria genotypes which were collected from western and southern parts of Turkey were improved by KNO3 and NaCl treatments. The experiment was conducted in two consecutive years, 2004 and 2005. Germination tests were performed at 15 degrees C and 18 degrees C. Results indicated that the effect of the treatments varied between the genotypes, temperatures and solutions that were used. KNO3 treatment was better and effective in larger number of lots than NaCl in both years. NaCl promoted germination in a few but inhibitive in large number of lots at both temperatures. As germination temperature was reduced, the positive effect of the KNO3 became greater. As a conclusion, priming treatments, specifically KNO3 appears to be promising in enhancing germination percentages of Lagenaria siceraria particularly at 15 degrees C that is the prevailing temperature in rootstock production for grafted watermelon seedling in unheated glasshouse conditions in early spring.Öğe Fistulized Pseudoaneurysm Associated with Hypothenar Hammer Syndrome to the Skin in a 12-Year-Old Patient: A Case Report(Korean Soc Vascular Surgery, 2023) Beyaz, Metin Onur; Kaya, Sefer; Demir, IbrahimIn this case, we present a condition where the extension of the hamate hook in the Guyon canal can damage the ulnar artery or its branches, leading to the development of an aneurysm or pseudoaneurysm. The patient, a 12-year-old female, presented to our clinic with a complaint of an uncontrolled palm lump that has been growing for several months and began to bleed in a pulsatile manner after trauma. She was an amateur volleyball player who trained twice weekly for two hours. Color Doppler ultrasound examination revealed a 1.1x0.8 cm pseudoaneurysm in a branch of the ulnar artery. Aneurysmectomy and primary repair were performed. Timely diagnosis and treatment planning are crucial for ulnar artery pseudoaneurysms or aneurysms to prevent ischemic events in a later period.Öğe Foreign body embolization by the aortic root due to a fire gun injury: A case report of a 10-year-old asymptomatic patient(Mexican Acad Surgery, 2022) Kaya, Sefer; Oruc, Dilan; Anil-Unal, Ekin; Demir, Ibrahim; Onur-Beyaz, MetinForeign body embolies that origin arterial system have mostly seen symptomatic and that's why they require emergency surgery. However, asymptomatic cases are followed conservatively, they can cause vascular-related end-organ pathologies as time passes. Foreign body migration from the ascending aorta to the end-organ artery is a very rare in literature after a fire gun injury. In symptomatic cases, emergency surgery should be planned immediately. In asymptomatic cases, the risks and benefits should first be considered before any surgical approach.Öğe Management of Nonocclusive Mesenteric Ischemia in Patients with Cardiac Failure(Forum Multimedia Publishing, Llc, 2022) Omeroglu, Sinan; Tanal, Mert; Beyaz, Metin Onur; Guven, Onur; Demir, Ibrahim; Gurbulak, Esin Kabul; Demir, UygarAim: This study aimed to evaluate the surgical procedures, outcomes, and prognostic factors in patients with ischemic heart disease who were operated on due to nonocclusive mesenteric ischemia (NOMI). Material and methods: This research contains all patients diagnosed with congestive heart failure and NOMI between January 2011 to January 2020. The patients who had angiography or CT that showed occlusion of more than 50% in any of the main branches of the mesenteric artery or patients who presented with symptoms in correlation with a total occlusion were excluded from the study. Patients who underwent coronary heart surgery but were not diagnosed with congestive heart failure and those with atrial fibrillation also were excluded from the study. Patients divided into two groups, according to a medical database. Results: A significant difference was found between the surviving and non-survivor groups in minutes, in terms of median time to segmenter intestinal resection (P = 0.042). Conclusion: An early diagnosis and surgical segmental intestinal resection before peritonitis worsens can be the key to better prognosis for NOMI patients.Öğe The radicle emergence test and storage longevity of cucurbit rootstock seed lots(Ista-Int Seed Testing Assoc, 2022) Ermis, Sitki; Oktem, Guleda; Mavi, Kazim; Hay, Fiona R.; Demir, IbrahimA study was carried out to test whether a single count of radicle emergence (RE test) would correlate with the storage potential of seed lots of cucurbit (C. maxima x C. moschata) rootstock cultivars. The RE test was performed by counting radicle emergence percentage between 26 and 100 hours of germination. Seed longevity was determined by storage at 75% relative humidity at 35 degrees C over 100 days, with seed survival curves constructed based on normal germination percentages. K-i (estimated initial viability in probits), sigma (standard deviation of the normal distribution of seed death in time) and p(50) (time for viability to fall to 50%) were determined through probit analysis. Correlation analysis showed that RE counts particularly between 30 and 34 hours were highly (P < 0.01) correlated with normal germination percentage, K-i and p(50). It can be concluded that RE tests can be used as an indicator of subsequent seed lot longevity in cucurbit rootstock seed cultivars.Öğe Surgical Treatment of Asymptomatic Popliteal Artery Aneurysms and Mid-term Outcome(Springer India, 2021) Beyaz, Metin Onur; Ata, Emin Can; Demir, Ibrahim; Onalan, Mehmet Akif; Sayin, Omer AliPopliteal artery aneurysm (PAA) is a rare condition with an incidence ranging from 0.8 to 2.8%; however, it constitutes approximately 70-85% of all peripheral artery aneurysms. It is asymptomatic in the majority of cases but can cause pain and edema due to venous and neuronal compression. The most severe complication is limb lost due to thromboembolic event. Although surgical treatment left its place to endovascular treatment in the 1990s, surgical treatment still maintains its importance in preventing complications. Here, we aim to report our experience and results of the surgical management of popliteal aneurysms in this study. In this retrospective study, a total of 21 patients who were operated on due to popliteal artery aneurysm between October 2017 and January 2020 were analyzed. Patients with pseudoaneurysm and those who are infected and complicated were excluded from our study. The mean age was 63.3 +/- 9.6; females were 17 (81%). Mean follow-up was 19 +/- 8 months. More than two risk factors were found in 14 (66.7%) patients. Aneurysmectomy was performed successfully in all patients. Autologous saphenous vein graft was used in 11 (52.4%), whereas 6-mm polytetrafluoroethylene (PTFE) graft was preferred in 10 (47.6%) patients. During the follow-up period, limb loss rates were 16.7% and 83.3%, respectively. The difference was statistically significant (p < 0.05). No relation was found between the aneurysm diameter and postoperative graft occlusion. Limb loss rate was high in popliteal aneurysm repair using PTFE graft due to graft occlusion; saphenous vein graft is more superior in terms of mid-term graft patency.Öğe Upper extremıty arterıal aneurysms: Etıology, management, and outcome(Mexican Acad Surgery, 2022) Onur-Beyaz, Metin; Demir, Ibrahim; Can-Ata, EminBackground: Upper extremity arterial aneurysms are not common clinical conditions. It may causes ischemic gangrene or limb losses as a result of thromboembolic events due to endothelial damage. In this study, we aimed to investigate the etiology, management, and long-term outcome. Methods: A total of 55 upper extremity aneurysms between January 2009 and April 2018 were retrospectively investigated. The mean age was 41 +/- 13 years, and the women and men were 13 (23.6%) and 42 (76.4%). The mean follow-up was 43 +/- 18 months. Results: The incidence of pseudoaneurysm was higher than the true aneurysm (64.5% vs. 35.5%). 30 (84%) patients who had pseudoaneurysm repaired primerely and 6 (16%) patients had patchplasty. About 58% of the true aneurysm caused by blunt trauma. 4 (21%) true brachial aneurysm patients were required emergency intervention due to distal ischemia. 16 (84.2%) patients operated using by saphenous vein graft. The primary and secondary patency was 87.5% and 93.8%. No limb or life loss occurred during follow-up. Conclusion: Pseudoaneurysms are more common among upper limb aneurysms and they occur mostly by iatrogenic causes. Blunt trauma can be main cause of the true aneurysm. Surgical resection of the aneurysm and interposition of saphenous vein graft provides excellent results in the long-term.
