Öğe The efficacy of intrauterine versus oral progestin for the treatment of endometrial hyperplasia. A prospective randomized comparative study(Imr Press, 2013) Dolapcioglu, K.; Boz, A.; Baloglu, A.Objectives: This study compared the efficacy of the levonorgestrel-releasing intrauterine device (LNG-IUD) to oral medroxyprogesterone acetate (MPA) applied for the same length of time for the management of endometrial hyperplasia without atypia. Study Design: This was single-center, open, randomized, and clinical trial. One hundred four patients aged between 30-50 years and diagnosed with endometrial hyperplasia without atypia by endometrial biopsy, were randomized to receive LNG-IUD or MPA. Both groups were further divided into two groups as three-month and six-month treatment subgroups. The primary objective was to compare the complete regression rates of hyperplasia, and the secondary objective was to determine the minimum duration of time required for the achievement of regression. Results: At two-year follow-up, the success rates of LNG-IUD treatment and oral MPA for three months therapy were 84% and 50%, respectively. While the regression rate was 100% in the six-month LNG-IUD group, it was 64% in the oral MPA group. LNG-IUD appeared to have a significantly higher success rate (p = 0.0001). Conclusion: It is believed that by this study LNG-IUD applications may be a reliable preference for younger patients who wish to preserve their uterus and especially for non-atypical cases, and if the patient demands fertility, even a six-month application will provide effective treatment.Öğe MMP7 and YKL-40 expressions in endometrial endometrioid carcinomas(I R O G Canada, Inc, 2019) Tas, Z. A.; Ozgur, T.; Yaldiz, M.; Dolapcioglu, K.Purpose of investigation: The expressions of YKL-40, MMP-7 and their relations with prognostic factors in endometrioid endometrial carcinomas (EEC) were investigated. Materials and Methods: Paraffin blocks of 50 patients diagnosed with EEC, 27 patients with endometrial hyperplasia, and 26 patients with normal cyclic endometrium were determined and the immunohistochemical staining (IHS) results of MMP-7 and YKL-40 antibodies were obtained. These results were compared with prognostic parameters with statistical evaluation. Results: Score 1 immunostaining was observed in 13 (26%), score 2 in 12 (24%), score 3 in 9 (18%), score 4 in 5 (10%) with MMP-7 in EEC. There was no MMP-7 expression in 9 (% 18) cases in carcinoma group. There was no statistically significant difference in proliferative and secretory phases in normal cycle group (p = 0.527). There was also no statistically significant difference between the non-typical and atypical hyperplasia/EIN cases (p = 0.42). Positive immunostaining was obtained in 15 (58%) of 26 normal cyclic endometrium, in 21 (78%) of 27 hyperplastic endometrium, in 19 (38%) of 50 EEC with YKL-40. The differences of YKL-40 expressions among all the groups were statistically significant (p = 0.01)). MMP-7 and YKL-40 expressions were compared with clinicopathologic parameters in the carcinoma group and found no statistical significant difference (p > 0.05). Conclusion: The present findings were limited but suggested that MMP-7 and YKL-40 might play a role in tumor progression.Öğe Multiple uterine angioleiomyomas mimicking an ovarian neoplasm: a case report(Imr Press, 2009) Hakverdi, S.; Dolapcioglu, K.; Gungoren, A.; Yaldiz, M.; Hakverdi, A. U.Angioleiomyoma is a rare benign neoplasm that originates from smooth muscle cells and contains thick-walled blood vessels. A 69-year-old woman had been complaining of progressive abdominal distention for the last five years. At the exploration, a tumoral mass that could not be differentiated as to whether it had its origin from the uterus or ovary was specified. The patient underwent total hysterectomy and bilateral salpingo-oophorectomy (TAH+BSO). On pathologic examination of the specimen, the tumor was diagnosed as an angioleiomyoma. Here, we present a case of giant-multiple uterine angioleiomyomas mimicking an ovarian neoplasm.Öğe Reptile bite in pregnancy(Sage Publications Ltd, 2008) Duru, M.; Helvaci, M. R.; Peker, E.; Dolapcioglu, K.; Kaya, E.Reptile bites are rarely seen cases in emergency departments. But particularly in the warm summer months, the prevalence is relatively higher in rural areas. Snakebites during pregnancy is little known and only a few cases have been reported in the literature. However, in case of pregnancy, the outcomes may he different both for mother and the fetus. Interestingly, there is limited number of reptile bite cases during pregnancy in the literature. We report here a woman who was bitten at her 28th week of gestation. Obstetric consultation with ultrasonography was performed and seen that fetus was normal according to a 28-week gestation. We infused four vials of antiserum in a period of 1 h in a saline solution. In all, 30 min after the initiation of the antiserum infusion, the lesion started to regress. Her obstetric consultation was repeated 1 h, 36 h, and before discharge were performed and seen that the fetus was normal. After a period of five days, the patient and fetus were evaluated once more, and nothing was abnormal for both.
