Öğe Bir üniversite hastanesi çalışanlarında obezite görülme sıklığının ve beslenme alışkanlıklarının araştırılması(2016) Canbay, Özden; Doğru, Esra; Katayıfçı, Nihan; Duman, Fatma; Şahpolat, Musa; Kaya, İnci; Dağ, Ersin; Kuş, KudretAmaç: Çalışmanın amacı, sağlık çalışanlarında obezitenin görülme oranının belirlenmesi ve obezitenin depresyon ile fiziksel sağlık üzerindeki etkisinin araştırılmasıdır.Gereç ve Yöntem: Araştırma Mustafa Kemal Üniversitesi Araştırma Hastanesi sağlık personeli ve yardımcı personele uygulanmıştır. Çalışmaya 131 birey dahil edildi. Sosyodemografik bilgi formu, New York postür analizi ve Beck depresyon ölçeği uygulandı. Bireylerin Vücut Kitle İndeksi (VKİ), günlük çalışma saatleri, günlük öğün sayıları, vücut imajı algısı, beslenme alışkanlıkları ve uyku durumları sorgulandı.Bulgular: Değerlendirmeye 19-60 yaş aralığındaki bireyler alındı. Değerlendirilmeye alınan 131 bireyin; 52'si erkek, 79'u bayandı. Bu bireylerin 85'i evli, 46'sı bekardı. Yemek masası başında zaman harcama süresi ile obezite arasında istatistiksel olarak anlamlı ilişki saptanmadı. Algı ile VKİ arasında istatistiksel olarak anlamlı farklılık bulundu. Bir öğünde aşırı tüketilen yiyecek sayısı ile obezite arasında istatistiksel olarak anlamlı ilişki saptandı. Yemek yerken su içme ve yemekleri az çiğneme ile VKİ arasında istatistiksel olarak anlamlı farklılık bulunmadı. Obez kişilerde asitli içecek tüketim sıklığı diğer bireylere göre istatistiksel olarak anlamlı daha fazla olduğu bulundu. Meyve-sebze tüketim sıklığı ile VKİ arasında istatistiksel anlamlı farklılık bulundu. Obez ve şişman olan bireylerin normal ve zayıf olan bireylere göre daha sık meyve tükettiği gözlemlendi. Yağlı şekerli besin tüketim sıklığı obez ve şişman bireylerde zayıf ve normal bireylere göre istatistiksel olarak anlamlı farklılık saptandı. Yemek öğün sayısı ile VKİ arasında istatistiksel olarak anlamlı farklılık bulundu. Sonuç: Önemi ve tehlikesi giderek artan bir toplum sorunu olan obezite ile mücadelede temel kişiler olan sağlık çalışanlarında obezite sıklığı ve etkileyen faktörlerin belirlenmesi son derece önemlidir. Obezitenin postür analizleri sonucunda önemli ortopedik sorunları da beraberinde getirdiği düşünülmektedir Ayrıca kilolu veya obez olan bireylerin vücut imajı algısının problemli olması obezitenin biyo-psiko-sosyo-kültürel açıdan değerlendirilerek tedavi programlarının kapsamlı olarak oluşturulması gerektiği düşünülmektedir. Obezitenin sistemik hastalıklar özellikle kardiyak, diyabet ve ortopedik problemleri de beraberinde getirmesi sonucunda bireye özgü uygun egzersiz verilmesi gerektiği düşünülmektedir. Aksi durumda ciddi sakatlanmaların ve hayati durumun ciddiyetini önemli derecede etkileyeceğini düşünülmektedir.Öğe Change in the dimensions of the lumbar area muscles after surgery: MRI analysis(Kare Yayıncılık, 2020) Duman, Fatma; Serarslan, Yurdal; Öztürk, Fatma; Yücekaya, Bircan; Atçı, NesrinObjective: This study aims to assess the change in the dimensions of the lumbar muscles in patients with chronic lower back pain using Magnetic Resonance Imaging (MRI) and to determine pre/post effects of surgery. Methods: We enrolled 28 individuals (13F/15M; age: 45.39±11.56 years) whose L2-S1 muscle measurements were obtained using MRI, before and at follow-up 6-12 months after surgery. The control group comprising 37 individuals (18F/19M; age: 34.41±10.72 years) who had no lumbar pathology but for whom retrospective archive images were available. In the axial MRI analysis, the cross-sections of m.multifidus, mm.erector spinae and m.psoas major on both sides were measured with the 'closed polygon' technique. Results: The L2-3 and L4-5 levels of the m.multifidus on the right side, the L2-3, L4-5 and L5-S1 levels of the m.multifidus and the L5-S1 levels of the mm. erector spinae on the left side cross-sectional areas were significantly lower than the control group (p<0.05). The right-side m.multifidus and the left-side mm.erector spinae sectional areas were significantly lower than the pre-surgery values at the L5-S1 levels (p<0.05). Conclusion: This study demonstrated that chronic lower back pain causes atrophy in the lumbar muscles and established the existence and continuity of atrophy after surgery.Öğe A Comparative Analysis of Surface Areas and Ratio of the Cervical Spinal Cord and the Vertebral Canal at the Same Levels Via MRI on Healthy Individuals(Soc Chilena Anatomia, 2014) Duman, Fatma; Ziylan, Taner; Kiresi, Demet; Cicekcibasi, Aynur Emine; Buyukmumcu, Mustafa; Duman, TolgaThe determination of the normal values of the cross-sectional surface areas and ratios of the vertebral canal and the spinal cord on the healthy individuals is of great importance with regards to the fact that it provides convenience for the doctors to make correct pathological diagnosis because of the most suitable treatment. In this study, it has been aimed to determine the respective ratios between the vertebral canal and the spinal cord via measuring their cross-sectional surface areas at the C3-C6 vertebra levels. The study has been implemented on the Magnetic Resonance Images (MRI) of the cervical vertebral column from healthy individuals at the Department of Radiology, Meram Faculty of Medicine, Necmettin Erbakan University. The study has been applied to 67 (50 female-17 male) individuals whose ages varied between 14 and 59 and these individuals have been divided into two groups as below and over 40 years of age. During the cervical MRI examination, axial and sagittal images have been evaluated and measurements have been made on the non-pathological axial images. The vertebral canal and the spinal cord surface areas have been calculated in square millimeters at the C3-C6 levels. Also, we calculated the ratio between the spinal cord surface area and the vertebral canal surface area. The obtained data has been transferred on the computer and statistical analysis has been implemented via SPSS package program (for Windows, 15.0). The summary of the data has been stated as Mean +/- SD. It has also been compared with regard to sex and age groups (below and over 40 years of age) using the Student t-test. The relationship between parameters has been evaluated by means of Pearson correlation test. No significant discrepancy (P>0.05) has been determined between the male and the female subjects in terms of the vertebral canal and spinal cord surface area values. On the other hand, at the C4, C5 and C6 levels, a significant discrepancy (P<0.05) has been observed between male and female subjects in terms of the ratio of spinal cord surface to the vertebral canal surface, stating that the male have it larger than the female. A positive correlation (P<0.01) between the surface area and ratio data of both sexes has been determined. We believe that the results of this study will be useful for the fields of neurology, neurosurgery and physical therapy and rehabilitation, particularly in evaluating spinal atrophy.Öğe Evaluation of cardiorespiratory state in patients with cerebral palsy(İnönü Üniversitesi, 2019) El, Çiğdem; Hüzmeli, İrem; Doğru Hüzmeli, Esra; Duman, FatmaAim: The objective of this study is to compare the cardiorespiratory state of individuals with hemiparetic cerebral palsy and healthy individuals. Material and Methods: The demographic information of 40 individuals were evaluated. Then their body mass index and frequency and duration of physiotherapy were questioned. Gross motor skill levels with the Gross Motor Classification System (GMCS), flexibility with the sit and reach test, cardiorespiratory endurance with the 6-minute walking test and independence in daily life activities with the WeeFIM survey were evaluated. Oxygen saturation, respiration frequency and heart rate were recorded before and after evaluation. Results: 20 individuals with hemiparetic cerebral palsy (%55 female, %45 male) (with an age average of 9.7± 1.94) and 20 individuals as the control group (%65 female, %35 male) (with an age average of 9.1± 1.83) were included in the study. According to the sixminute test analysis, no important variety was observed between the two groups in terms with regard oxygen saturation, respiration frequency and heart rate parameters prior to and after the test (p>0.05). A statistically significant difference was observed between the patient and control groups in terms of walking distance (p=0.001) and the WeeFIM total score (p= 0.001) and flexibility test (p=0.001). Conclusion: Remarkably, walking distance, WeeFIM total score and flexibility test in patients with cerebral palsy can give an idea about prognosis and the adverse effects of disease in early period on patients’ physical, psychological and social development.Öğe Genital lenfödem : olgu sunumu(2018) Doğru, Esra; Duman, FatmaLenfödem, proteinden zengin interstisyel sıvının deri ve deri altı dokuda birikmesidir. On üç yaşında erkek hasta iki alt ekstremite ve skrotum ödemi şikayetiyle kliniğimize başvurdu. Hastanın tıbbi öyküsünden bir yaşındayken primer lenfödem tanısı konulduğu belirlendi. Sol alt ekstremite ve skrotuma lenf drenajı uygulaması ve bandajlama yapıldı. Sağ alt ekstremite için tedavi bitimine kadar hastaya varis çorabı giyilmesi önerildi. Birinci aşamada ödemi azaltmak için manuel lenf drenajı, cilt bakımı ve bandajlama uygulanırken, ikinci aşamada kompresyon çorabı ile tedavi desteklendi. On iki seanslık tedavi sonucunda ödemde önemli ölçüde ve hızlı bir azalma elde edildi. Materials and Methods: Research has been applied to the medical staff and auxiliary staff of Mustafa Kemal University Research Hospital. 131 individuals were included in the study. Socio-demographic information form, New York Posture Analysis and Beck Depression Scale were applied. Body Mass Index (BMI), daily working hours, daily number of meals, perception of body image, eating habits and sleep state were examined. Results: Individuals with age between 19-60 were included in the study. 52 individuals were male, 72 of them were female. Of these individuals, 85 were married, 46 were single. A statiscally significant relationship has not been found between spending time at the dining table and obesity. Statistically significant difference has been found between BMI and perception. Also, statistically significant relationship has been found between exessive consumption of food in a meal and obesity. There has been no significant relationship between drinking water while eating meal and less chewing meals. Acidic drink consumption frequency has been found to be more in obese individuals than others. A significant difference has been found between fruit-vegetable consumption frequency and BMI. It has been observed that obese and overweight individuals consume fruits more frequently than normal and slim individuals. Frequency of consumption of sugary and fatty foods has been seen more in obese and overweight individuals than normal and weak individuals. Statistically significant difference has been found between number of meals and BMI. Conclusion: Identification of the prevalence of obesity which is a society problem with increasing importance and factors affecting obesity in healthcare workers who are the main persons fighting against the obesity are extremely important. It has been thought that obesity brings significant orthopedic problems due to postural issues. Additionally, body image perception in overweight and obese individuals result in a biopsycho- socio-cultural aspects and treatment program must be created comprehensively. Obesity causes systemic diseases, especially cardiac, diabetes and orthopedic problems, so appropriate and specific exercises should be given individually.Öğe Poor myocardial blush grade accompanies to higher sudden death risk scores of hypertrophic cardiomyopathy(İnönü Üniversitesi, 2019) Akkuş, Oğuz; Kaplan, Mehmet; Duman, Fatma; Yalçın, FatihAim: Hypertrophic cardiomyopathy (HCM) is predominantly inherited as an autosomal dominant disease. The aim of our study was to evaluate angiographic microvascular flow and predictors of poor MBG in patients with HCM. Material and Methods: Patients with poor MBG (MBG 0/1, n=43), and patients with normal MBG (MBG 2/3, n=15) were included retrospectively. Results: Family history and left ventricular end-diastolic diameter were significantly higher in the poor MBG group (p = 0.025, p=0.021). Left atrium diameter and moderate to severe mitral regurgitation (MR) were significantly higher in the normal MBG group (p=0.034, p=0.002). Percentage of SCD risk was significantly higher in patients with poor MBG (p=0.014). Moderate and severe mitral regurgitation (odds ratio =0.013, p=0.004) and HCM-SCD risk score (odds ratio = 0,398, p=0.009) were found to be independent parameters for predicting poor myocardial blush grade. The cut-off value of HCM-SCD risk score obtained by ROC curve analysis was 3.10 for the prediction of poor myocardial blush grade (sensitivity: 76.2%, specificity: 76.7%). The area under the curve (AUC) was 0.712 (p=0.016). Conclusions: Our study results demonstrated the increased risk of sudden cardiac death accompanies poor myocardial blush grade in patients with HCM. Methods: Fifty (21 males and 29 females) slightly overweight and obese participants with no chronic disorder and of an average age of 12.16±0.99 years were included in the study. The percentile values for BMI, triceps skinfold thickness, waist circumference measurements, and physical performance tests were evaluated. The effects of obesity on psychological wellness were evaluated using the depression scale for children (DSC) and the Piers-Harris Children's Self-Concept Scale for selfesteem. Following these evaluations, the participants were subjected to an exercise program in five groups of 10 people, 3 days a week for a duration of 8 weeks. Each exercise session lasted 45 minutes. Participants were re-evaluated at the end of the exercise program. The data collected both before and after the exercise program were analyzed using the SPSS 18.0 program. Results: According to BMI reference values, 28% of the 50 participants (n=14; 6 males and 8 females) were assessed to be overweight and 72% to be obese (n=36; 15 males and 21 females). Following the exercise program, 14% of the participants (n=7; 3 males and 4 females) were assessed as normal, 46% (n=23; 14 males and 9 female) as slightly overweight, and 40% (n=20; 4 male and 16 female) as obese. It was determined that the decrease in BMI values (p<0.05) was higher in male participants than in female participants and that the frequency of obesity was higher in the females. A statistically significant decrease in BMI values was found after the exercise program (p<0.01). Following the exercise program, statistically significant differences have also been observed in the self-esteem (p<0.01), psychological wellness (p=0.025), triceps skinfold thickness, as well as in waist circumference and BMI values of the participants compared to the pre-exercise phase (p<0.01).
