    Anti-tumoral effect of beta-blockers on prostate and bladder cancer cells via mitogen-activated protein kinase pathways

    The incidence of prostate cancer in the world is increasing every year. Death caused by prostate cancer is increased by 13% in men between 1980 and 2005. It is the second leading cause of cancer death in men after lung cancer. Bladder cancer is the second most common of urological malignancies. Most of the bladder cancers are treated with transurethral resection. Even great efforts have been made in the treatment of bladder cancer over the past years, it still remains as a major health problem. New therapeutic approaches are required to prevent the development and metastasis of these diseases. Experimental and clinical studies have shown potential beneficial effects of co-administration of beta-adrenergic receptor antagonists (beta-blockers) during cancer therapy. This study aimed to investigate the anti-tumor activity of beta-blockers on prostate and bladder cancer. Prostate and bladder cancer cell lines were cultured and treated with beta-blocker (propranolol). Then, protein levels and activity of apoptotic pathway mediators and mitogen-activated protein kinase (MAPK) pathway mediators were analyzed by ELISA. Propranolol treatment elevated the activity of caspase-3 and expression of bax, Wee1, GADD153 and apoptosis-inducing factor, but decreased bcl-2 which is an antiapoptotic protein. Propranolol treatment also inhibited ERK and JNK activity. This study showed that propranolol will help to inhibit prostate and bladder cancer by activating apoptotic pathway and by inhibiting MAPK pathway. This is the first study investigating the apoptotic effect of propranolol via MAPK on prostate and bladder cancer.

    Association of diabetic retinopathy stage with the severity of female urinary incontinence

    Purpose To assess the relation between diabetic retinopathy (DR) severity and urinary incontinence (UI) in patients with diabetes mellitus (DM). Materials and methods This prospective and observational study included 153 subjects. Patients were divided into three subgroups, according to severity of DR, as: No-DR, nonproliferative DR (NPDR), and proliferative DR (PDR); 40 age-matched healthy subjects formed the control group. Turkish version of the Urogenital Stress Inventory 6 (UDI-6) and Incontinence Impact Questionnaire (IIQ-7) were used to assess the UI symptoms and their effect on quality of life. The UDI-6 and IIQ-7 scores were the primary outcomes of the study. Results No significant difference was observed between groups regarding age, maternal parity, body mass index, type of delivery, menopausal status, and smoking. The mean UDI-6 urgency UI questions score was significantly higher in the PDR group and significantly higher in the NPDR group than in the control group. The mean UDI-6 stress UI questions score was similar between groups. The mean UDI-6 voiding difficulty questions score was significantly higher in the PDR group and no significant difference was observed between other groups. The mean IIQ-7 score was significantly lower in the PDR group. A moderate and positive correlation was found between glycated hemoglobin level and the UDI-6 urgency UI and voiding difficulty questions and total scores. A weak and positive correlation was found between the duration of DM and the all UDI-6 scores. Conclusion The present study showed that UI symptoms and their effect on QOL were more severe in patients with PDR.

    Association of pupil responses with severity of erectile dysfunction in diabetes mellitus

    Purpose: To investigate the relation between erectile dysfunction (ED) severity and pupillary functions in patients with diabetes mellitus (DM). Methods: This prospective and observational study included 90 patients with type 2 DM and ED. Patients divided into three subgroups according to severity of ED: (i) Mild ED, (ii) Moderate ED and (iii) Severe ED groups. Thirty age-matched healthy subjects formed the control group. Main outcome measures were pupil diameter and average speed of pupil dilation. Static and dynamic pupillometry analysis was performed using the Sirius Topographer (CSO, Firenze, Italy). Results: Mean pupil diameter during static and dynamic pupillometry analysis were significantly greater in the control group than in the all study groups (P < 0.05). Mean pupil diameter in static pupillometry analysis was significantly different in each study group and pupil was more miotic in the Severe ED group than in the both Moderate and Mild ED groups (P < 0.05 for each). Dynamic pupillometry analysis revealed that mean pupil diameter and mean average dilation speed were significantly different in each study group throughout measurement period and the highest speed was observed in the Mild ED group and the lowest speed was observed in the severe ED group (P < 0.005 for each). Conclusion: Our study results suggest that abnormal pupil functions due to diabetic autonomic neuropathy may indicate the associated ED in patients with DM.

    Does Halitosis Effect Sexual Life in Males?

    Aim: Halitosis is an unpleasant alteration of the halitus. The aim of the study is to assess the quality of sexual life of males with and without halitosis. Material and Method: Males between 20-50 years old; with a complaint of halitosis; married and able to complete the study were included in the study. The control group were healthy, adult, married males who have normal oto-rhinolaryngologic examination. The International Index of Erectile Function questionnaire(IIEF) were used to assess the quality of sexual life of males with halitosis and the results were compared with a similar group of healthy men. Results: A total of 80 adult subjects completed the study. When the groups were compared in terms of age, body mass index(BMI) and duration of marriage, they were similar (p=0.65, p=0.20, p=0.08 respectively). The halitosis group had significantly lower scores for all function domains (p=0.0001). There was no correlation between degree of halitosis and the scores of IIEF (p<0.05). Discussion: This is the first study investigating the sexual health of males with halitosis. Halitosis not only effects oral health status but also strongly declines quality of life. The otorhinolaryngologist must not only treat halitosis but also help the patient to stand up to social and sexual problems concomitting halitosis.

    Impact of septoplasty on quality of sexual life of males with nasal septal deviation

    Aim: The aim of this study is to evaluate the quality of sexual life of males with nasal septal deviation (NSD). Also, we aimed to find out the impact of septoplasty operation on quality of sexual life. Material and Method: 30 patients with NSD and 30 control subjects with no nasal deformity were included in the study. International Index of Erectile Function Questionnaire (IIEF) was used to evaluate the quality of sexual life of the subjects. Results: The mean scores of erectile function, orgasmic function, sexual desire, intercourse satisfaction and overall satisfaction for NSD group were significantly lower than control group(p <= 0.05). The mean scores for all domains were significantly different before and after septoplasty in NSD group(p <= 0.05). Discussion: NSD seems to have negative effects on quality of sexual life in males. Also, we detected that a significant improvement in sexual performance can be provided by the treatment of nasal obstruction by septoplasty in patients with NSD.

    Influence of visual acuity level on sexual function in patients with cataract

    Purpose: To assess the association of sexual function with visual acuity. Methods: Patients were included in the mild to moderate visual impairment (MVI), severe visual impairment (SVI), and profound visual impairment (PVI) groups based on bilateral corrected distance visual acuity. All patients completed the Turkish version of the International Index of Erectile Function (IIEF) and Glombok-Rust Inventory of Sexual Satisfaction (GRISS) scales before and at 1 month after the cataract surgery. Results: Mean IIEF scores did not improve after cataract surgery in the MVI group (P > 0.05). Sexual desire, intercourse satisfaction, overall satisfaction, and total IIEF score were significantly improved after cataract surgery in both the SVI and PVI groups (P < 0.05). Mean GRISS subscale scores did not improve after cataract surgery in the MVI group (P > 0.05). Non-sensuality, avoidance, dissatisfaction, infrequency, noncommunication, and total GRISS score were significantly improved after cataract surgery in both the SVI and PVI groups (P < 0.05). Conclusion: The study results documented the association of low visual acuity and decreased sexual desire and satisfaction and revealed the beneficial effect of visual improvement.

    THE PREVALENCE OF URINARY INCONTINENCE, SEXUAL DYSFUNCTION AND QUALITY OF LIFE IN WOMEN OF REPRODUCTIVE AGE ADMITTED TO UROGYNECOLOGY DEPARTMENTS

    Aim: Urinary incontinence (Ul) is fairly common among women and reduces common emotional and psychological well-being and also a phenomenon that weakens sexual life. At the same time social activities and relationships; family life, his relationship with his Wife is adversely affected. In this study of prevalence of Ul and the sexual dysfunction(SD) and quality of life. were examined in women of reproductive age who admitted family medicine, obstetrics, urology clinics With various symptoms. (frequent urination; urinary burning; pelvic pain, urinary incontinence, vaginal discharge and similar complaints). Materials and methods: The study included a total of 665 female patients of 15-49 years of age admitted to Family Medicine, Obstetrics and Gynecology, and Urology polyclinics in Mustafa Kemal University Faculty of Medicine, Training and Research Hospital during October 2013-January 2014 with various complaints. General survey form ICIQ-SF (International Consultation on Incontinence Short Form), FSFI (Female Sexual Function Index) form; and EORTC QLQ-C30 (European Organization for Research and Treatment of Cancer Quality of Life Questionnaire C30) version 3.0 were applied to these patients The general survey form addressed demographic characteristics, risk factor for urinary incontinence, concomitant diseases, anamnesis including medical history and family history. The data obtained in the study were assessed for statistical significance by using Student's (t-test, Mann Whitney U test, Chi,square test and Fisher's Exact Chi-square test. Results: The sociodemographic characteristics of the women were; mean age was 31.85 +/- 9.495 (SD) and mean body mass index (BMI) value was 26.39 +/- 4.03. Obstetric history was as follows; number of normal vaginal births was 317 (47,66%);number of caesarean sections 471 (70.8%); number of deliveries of infant over 4 kg of birth weight 138 (20.8%) and number of multiple gestations resulting in live birth was 60(9%) 235(35 3%) women had previous history of surgery to pelvic floor and 203 (30,5%). of had previous history of abdominal surgery (except cesarean section). Frequency of Ul was 42,7%(284 women) Situation that provokes Ul; mixed type at 95 (143%) women, stress type at 133 (20%) women, urge type at 56 (8.4%) women. The effect on daily life was scored(those who report an effect of 8 on a scale to 10 was 159 (23.9%) of). There was chronic disease at 175(26,4%) of them, disc herniation at 36 (5.4%) of, involuntary defecation at74 (11.1%)of, involuntary flatulence at 191 (28.7%) of, haemorrhoids at 405 (60.9%)of urinary tract infection at 244 (36,7%), vaginitis 1-2 times a year at 352 (52.9%) women. Sexual dysfunction (FSFI according to survey results); 215 (32.3%) Women have-not heard of sexual desire in the past month. Self-confidence about being sexually driven present at only in 222 (33.4%) women. 307 (46.2%) women dont feel wetness during sexual intercourse. 136(20.5%) women say sexual intercourse is not satisfactory. 57.9% of all cases not satisfied with your sexual life. Painful intercourse at 74 (11.1%) women. Conclusion: Urinary incontinence in female population is quite a common health problem. A large majority of women perceive it as a result of aging and female gender and do not seek medical assistance it is important to detect this problem and relevant risk factor among patients who admit to palyclinics. This approach would improve the treatnient of this condition which affects quality of life and sexuality.

    Quality of Sexual Life in Males with Allergic Rhinitis

    Aim: The aim of this study is to evaluate the quality of sexual life of males with allergic rhinitis(AR). Material and Method: 40 patients with AR diagnosed with skin prick test and 40 control subjects with no evidence of allergy completed the study. International Index of Erectile Function questionnaire (IIEF) was used to evaluate the quality of sexual life of the subjects. Results: The mean scores of erectile function, orgasmic function, sexual desire, intercourse satisfaction and overall satisfaction for AR group were significantly lower than control group(p= 0.0001). Discussion: AR has negative effects on quality of life. The effective treatment of AR by the clinician will also avoid these concomitting social, sexual and sleep disturbances.

    Quality of Social and Sexual Life in Males with Hearing Loss

    Objective: Hearing loss(HL) is a very common medical condition that leads to a decline in quality of social and sexual life. This study aimed to assess the quality of social and sexual life in male patients with HL. Materials and Methods: We studied two groups: (1) adult men with acquired, bilateral, sensorineural hearing loss(SNHL), and (2) healthy, adult, married men that have normal hearing levels, as the control group. Quality of social life was assessed using the The Hearing Handicap Inventory for Adults (HHIA) and 36-Item Short Form Health Survey (SF-36) and quality of sexual life was assesssed using the International Index of Erectile Function(IIEF) questionnaire. Results: There was statistically significant differences between the groups regarding the scores for HHIA,SF-36 and IIEF questionnaire. Conclusion: Our results indicate that men with SNHL have poorer social and sexual life.

    Spinal anesthesia for a rare case of Proteus syndrome

    Our purpose is to present an uneventful spinal anesthesia experience in a rare case with Proteus syndrome (PS) suffering from urolithiasis. A 17-year-old male patient with previously diagnosed PS admitted to the urology polyclinic for urolithiasis. Physical examination showed disproportionate growth of the lower extremities, amputated toes and superficial and large veins in the legs. Spinal anesthesia was successfully administered at sitting position through the intrathecal route via the L3-4 space in the midline. Since it also includes difficult airways, spinal canal deformities, thromboembolisms, and certain pathologies such as cystic abnormalities in the lung, emphysema, atelectasis and fibrosis, this syndrome should be considered further with respect to anesthesia. The most significant cause of mortality in PS is pulmonary thromboembolism induced by deep vein thrombosis caused by growing extremities. Spinal anesthesia has provided a more reliable method of anesthesia due to the lower risk of development of thromboembolism in this case.
