Amitriptilin zehirlenmesi, çeşitli sistemleri etkileyen yan etkileri nedeniyle önemli sorunlara yol açmakta ve hayatı tehdit edebilmektedir. Genellikle ölüm sebebi kardiyak etkilenmeye bağlı olmaktadır. Bu yazıda, yüksek dozda amitriptilin almasına rağmen EKG’de QRS değişikliği göstermeyen ve kardiyak etkilenmenin olmadığı bir olgu sunulmaktadır.Öğe From darkening urine to early diagnosis of alkaptonuria(Medknow Publications, 2008) Peker, Erdal; Yonden, Zafer; Sogut, SadikAlkaptonuria is a rare disorder of metabolism characterized by deficiency of homogentisic acid oxidase. Characteristic features include darkening of urine, ochronosis, and arthropathy. Darkening of urine is the only sign of the disorder in the pediatric age group, and it occurs at very early stage of the disorder, as reported by the parents. A 4-year-old boy presented to our clinic with the complaint of dark urine and bluish black staining of clothes. This darkening pointed to a positive physical history of bluish discoloration of sclerae which occurred off and on. We initiated treatment with ascorbic acid and a protein diet with restriction of phenylalanine and tyrosine (1.6 g/kg/d). The subjects were selected from those who visited for a checkup or medical treatment at the Mustafa Kemal University Research Hospital. Serum levels of NO-2, NO-3, malondialdehyde (MDA), superoxide dismutase (SOD), and catalase (CAT) were analyzed by redox or enzymatic reactions and spectrophotometry. Based on the NO-2 and NO-3 levels, the NO concentration was about 10% higher in VPA group than in the control group (p < 0.001). However, no significant difference was detected for serum MDA, SOD, and CAT levels. It is suggested that NO would play a role in the mechanism of antiepileptic effects by VPA treatment. Children are at greater risk of developing severe cardiac, respiratory, and neurological complications due to lesser body surface area. Alpha receptor stimulation plays important role in the pathogenesis of pulmonary edema. Prazosin, a post synaptic alpha blocker, can be recommended as an effective drug in the treatment of serious scorpion envenomations with significant sympathetic symptoms. Oral prazosin is fast acting, easily available, relatively cheap, free from any anaphylaxis and highly effective.Öğe A rare tuberculosis form : Congenital tuberculosis(2010) Peker, Erdal; Bozdoğan, Erol; Doğan, MuratCongenital tuberculosis is defined as infection developing as a result of the encounter between the infant and Mycobacterium tuberculosis bacilli during the intrauterine period or during normal birth. Although tuberculosis infection is very common all over the world congenital tuberculosis cases are rare and mortalities of 50% have been reported. Non-specific symptoms in congenital tuberculosis and difficulties encountered in the diagnosis of tuberculosis in general, make it difficult to reach a final diagnosis. Cases of congenital tuberculosis are generally known clinically during the first postnatal month. This case is important as the three-month infant was presented with pneumonia symptoms and diagnosed as congenital tuberculosis after an attentive anamnesis which is unusual during three postnatal months. To our knowledge, this case is one of a few cases in the literature diagnosed as congenital tuberculosis in three months. Our case emphasizes that it is necessary to consider congenital tuberculosis in the differential diagnosis of pulmonary infections in infants, particularly in countries where the incidence of tuberculosis is high. For this reason, we believe that a successfully obtained anamnesis is the parameter which provides a valid diagnosis. Human herpesvirus (HHV)-6 reactivation is an accepted criterion for the diagnosis of DINS and has prognostic importance. We report a 15-year-old boy with juvenile rheumatoid arthritis who developed DINS due to sulfasalazine, associated with reactivation of HHV-6.
