    Bone cutting performance and mechanical properties of piezo-surgical tips: a nano-indentation study

    Purpose: The aims of this study were to compare the mechanical properties of piezo-surgical tips such as nano -hardness, elastic modulus, surface roughness, and wear level, and to measure their cutting performance. Materials and Methods: In this study, 31 piezo-surgical tips were used, three for control and 28 for testing. The testing tips were equally divided into four groups with different numbers of osteotomies: the four-, 8-, 16-, and 32-osteotomy groups. The mean osteotomy duration was recorded during osteotomy. Scanning electron microscopy images of the tips in the test groups were obtained before and after osteotomy, and the wear level of the tips was measured. Results: A statistically significant increase was observed in the nano-hardness of the piezo-surgical tips depending on the number of osteotomy (for 4-use; 22.47 +/- 1.67H and for 32-use; 28.49 +/- 3.42H). The elasticity value of the testing tips was in the range of 218.55 +/- 15.74E to 241.26 +/- 10.46E, and all of the values were significantly higher than those in the control group (174.39 +/- 13.53E). As the frequency of use increased, a significant increase in surface roughness was observed (from 16.67 +/- 1.50 to 56.12 +/- 2.60). A positive correlation was found between the frequency of use and the wear level of the tips, and between the surface roughness and wear level of the tips. Conclusion: With the increase in the number of osteotomies, significant changes in the mechanical and physical properties of the piezo-surgical tips that affected their bone-cutting performance were observed.

    Comparison of Ultrasonography and Cone Beam Computed Tomography in the Differential Diagnosis of Periapical Lesions: A Prospective Radiopathological Study

    Objective: The aim of this study is to evaluate the correlation between ultrasonography (USG) and cone beam computed tomography (CBCT) and the accuracy between histopathological diagnosis and preliminary diagnosis in the diagnosis of periapical lesion. Methods: 20 patients with periapical lesion in the jaw, were included in the study. The presence of expansion or perforation and dimensions of the lesion were performed with CBCT. In the examination of the lesion with USG, shape, echogenicity, vascularization of the lesion and the presence of buccal expansion and perforation, were determined. Subsequently, a biopsy was taken from the lesion for histopathological examination and the final result was compared with the accuracy of the preliminary diagnoses. Results: Kolmogorov-Smirnov test, Wilcoxon test (w) and Cohen's kappa coefficient (kappa) was used to analyze the data. Three of the 4 lesions diagnosed as periapical granuloma as a preliminary diagnosis were confirmed as periapical granuloma in histopathological examination. Periapical cyst was confirmed in histopathological examination of 14 of 16 lesions diagnosed as periapical cyst as a preliminary diagnosis. Mesiodistal (MD) measurements in CBCT measurements were significantly higher than the USG group (p <0.05). There was 100% agreement (p = 0.000) between the evaluation of buccal expansion, buccal perforation, and palatal-lingual perforation between CBCT and USG. Conclusion: It was concluded that the combined use of USG and CBCT can provide the clinician with important information in the diagnosis of periapical lesion.

    Protective Effect of Hypericum perforatum Extract on Methotrexate-Induced Osteotoxicity via Reducing Oxidative Stress and MAPK Activity

    Backgrounds and Objective: Methotrexate (MTX) is an anti-metabolite and anti-rheumatic drug that is widely used in cancer treatment. It is known that a high dose of MTX chemotherapy causes bone growth disturbance in developing bones. Hypericum perforatum (HP) plant is useful against some diseases for local people and regulates apoptosis which is also known as programmed cell death and recent studies also reported that HP reduces the side effects of many toxic drugs. For this reason, we planned a study to reveal the osteoprotective effect of HP extract against MTX induced osteo-toxicity. Materials and Methods: For this purpose MLO-Y4 cells were cultured, then these cells were treated with HP and methotrexate and mediators of the apoptotic, MAPK pathway and the oxidative stress parameters were analyzed on these cells. Results: Expression of proapoptotic factors increased and expression of antiapoptotic factors were decreased with MTX treatment. Treatment of HP ameliorated the apoptotic effect of MTX on MLO-Y4 cells. HP treatment also inhibited MAPK activity and oxidative stress induced by methotrexate. Conclusion: HP attenuated methotrexate-ind uced osteo-toxicity via reducing oxidative stress and MAPK activity and HP is an effective reducing side effect of MTX.
