    An atypical bladder diverticulum presented with recurrent peritonitis : Case report

    Mesane divertikülleri detrusor kasının doğuştan zayıflığından kaynaklanır. Sıklıkla divertikül içindeki üriner staz sunucu gelişen idrar yolları enfeksiyonu ile kendini gösterir. Mesane çıkım tıkanıklığı, alt ekstremitede siyanoz, intestinal tıkanıklık, üreteral tıkanıklık gibi divertikülün doğrudan basısı sonucu ve divertikülün kendiliğinden rüptüre sonucu gelişen peritonit gibi farklı klinik tablolara neden olabilmektedir. Bu yazıda, perfore apandisiti taklit eden ve perforasyon olmadan tekrarlayan jeneralize peritonite neden olan mesane divertikülü olan bir olgu sunuldu.

    An atypical bladder diverticulum presented with recurrent peritonitis: case report

    Bladder diverticula develop from congenital detrusor muscle defect and frequently present with urinary tract infection, which occurs as a result of urinary stasis in the diverticula. Different clinical presentations, such as bladder outlet obstruction, cyanosis of the lower extremities, intestinal obstruction, ureteral obstruction (which may occur due to direct diverticular compression), and peritonitis due to spontaneous rupture of the diverticula, were reported previously. Here, we report a case with the diagnosis of bladder diverticulum that caused recurrent generalized peritonitis without perforation and mimicked perforated appendicitis.

    Effect of mesalamine on healing in experimental colon anastomosis: A randomised experimental study

    Objective: We aimed to investigate the effect of mesalamine on healing of experimental colon anastomosis model. Material/Methods: Forty adult male Wistar albino rats were performed segmentary colonic resection and end-to-end anastomosis. Animals were randomly divided into four groups: group I, anastomosis group, received no treatment (GI, n = 8); group II, anastomosis + oral mesalamine group (100 mg/kg/day); group III, anastomosis + rectal mesalamine (2 mL) group, (GIII, n = 8); group IV, anastomosis + oral mesalamine + rectal mesalamine (GIV, n = 8) group. A sham group (n = 8) was constituted and was performed laparotomy. Bursting pressure, hydroxyproline levels and histopathological characteristics of the anastomosis were analyzed. Results: Although it was not statistically significant, there was an increase in the burst pressure of the mesalamine group. When hydroxyproline measurements were compared there were statistically significant difference between the non-treated colon and all groups. There were significant differences between GI and GIII-GIV, GII and GIV. The differences between group I and II and group II and III were not statistically significant. When we compared the median amount of the histopathological changes, we found significant difference between the anastomosis and the mesalamine groups (P < 0.05). But when mesalamine groups were compared with each other we did not observe a significant difference. Conclusion: Mesalamine had positive effects which were not statistically significant on bursting pressure and statistically different significant effects on hydroxyproline (HP) levels based on the way of administration and statistically significant positive effects on histopathologic anastomotic healing in experimental anastomosis model. © 2007 Surgical Associates Ltd.

    Effect of propolis on healing in experimental colon anastomosis in rats

    Introduction: Propolis is the generic name for the resinous substance collected by honeybees, which is known to have antioxidant, anti-inflammatory, apoptosis-inducible effects. Anastomotic dehiscence after colorectal surgery is an important cause of morbidity and mortality. We aimed to assess the effect of propolis on healing in an experimental colon anastomosis in rats. Methods: Forty adult male Wistar albino rats were randomly assigned into 5 treatment groups with 8 rats in each: Group I, anastomosis+no treatment; Group II, anastomosis+oral propolis ( 600 mg/ kg/ d); Group III, anastomosis+oral ethyl alcohol ( 1 cc/ d); Group IV, anastomosis+rectal propolis ( 600 mg/ kg/ d); Group V, anastomosis+rectal ethyl alcohol ( 1 cc/ d). The bursting pressures, hydroxiproline levels and histopathological changes in each group were measured. Results: When bursting pressures were compared between groups, we observed that they were increased in the groups treated with propolis in contrast to all other groups. Hydroxiproline levels in the propolis groups were also significantly increased in contrast to the other groups. There was also a statistically significant difference in histopathological changes between the treatment types. When propolis administration methods were compared, we did not observe a statistically significant difference. Conclusion: Propolis has a significantly favourable effect on healing in experimental colon anastomosis, independent from the method of administration.

    Esterified hyaluronic acid improves cartilage viability in experimental tracheal reconstruction with an auricular graft

    OBJECTIVE: The aim of this study was to determine the efficacy of esterified hyaluronic acid (HYAFF) on the vitality of auricular cartilage grafts used for tracheoplasty, with respect to macroscopic and microscopic parameters. STUDY DESIGN: Prospective, controlled. SETTING: Academic research laboratory. SUBJECTS AND METHODS: The study included 14 New Zealand rabbits acquired specifically for the study. The rabbits were divided into two groups: the control group, in which free cartilage grafts were not exposed to any materials or additional procedures (n = 7), and the hyaluronic acid (HA) treatment group, in which auricular grafts and anastomosis lines were covered with HYAFF (n = 7). Free auricular cartilage grafts used for the reconstruction of experimentally created tracheal defects were anastomosed extraluminally. All the rabbits were sacrificed two months post surgery. Samples were collected and examined histopathologically. The sections were stained with hematoxylin-eosin, Masson's trichrome, and inducible nitric oxide synthase (iNOS) and examined under a light microscope. RESULTS: Fibrosis and cartilage mass significantly differed between the control and HYAFF group (P < 0.05). Immunohistochemical examination showed that more chondrocytes stained with iNOS in the control group than in the HYAFF group, according to histologists' observations. CONCLUSION: HYAFF catalyzed wound healing with less fibrous tissue formation, had chondroprotective and stimulatory effects on chondrocyte metabolism, and decreased nitric oxide production and apoptosis via improving the nourishment of free auricular cartilage grafts, subsequently preventing hypoxia and oxidative stress, particularly in the early postimplantation period.     A giant pulmonary hydatid cyst causing respiratory failure: Case report

    The clinical signs of hydatidosis depend on the size and location of the cyst. Small pulmonary cysts are usually asymptomatic unless it they show complications such as perforation and enfection. However, large cysts may be symptomatic by compression of the adjacent vital organs. We report a 6-year-old girl with huge hidatid cyst in the left hemithorax, who required emergency surgical intervention because of respiratory distress. © 2010 Düzce Medical Journal.

    A new term: complete preputial agenesis

    Preputium is the extended part of the foreskin of the penis and covers the glans penis. Several congenital anomalies of the preputium such as disproportionate preputial growth, epidermoid cyst, and phimosis have been reported previously. Although lack of the ventral part of preputium is associated with hypospadias, there are no articles about complete agenesis of the preputium. Here, we report a case with the diagnosis of complete preputial agenesis. This case is the first in our knowledge. (c) 2007 Elsevier Inc. All rights reserved.

    Orchiectomy or testosterone receptor blockade reduces intestinal mucosal damage caused by ischemia-reperfusion insult

    The aim of the present study was to investigate whether orchiectomy or administration of flutamide an antagonist of the testosterone receptor can reduce oxidative stress and histologic damage in the rat small bowel subjected to mesenteric ischemia/reperfusion (I/R) injury. A total of 32 Sprague-Dawley rats were divided into four groups. Group 1 was control (sham), group 2 was I/R, group 3 was I/R plus orchiectomy (orchiectomy was performed 14 days before I/R), group 4 was I/R plus flutamide (flutamide was given throughout 14 days before mesenteric IR). Rats were subjected to 45 min of mesenteric ischemia followed by 3 h of reperfusion. The levels of ileal malondialdehyde (MDA) and nitric oxide (NO) were found to be significantly lower in orchiectomy and flutamide treatment groups compared with I/R group (P < 0.05). The histopathological injury scores were consistent with the MDA and NO levels. These results suggest that castration or testosterone receptor blockade decreases the level of intestinal I/R injury in male rats and it is an another example for disease variations based on gender differences.

    Solitary rectal ulcer causing lower gastrointestinal bleeding is extremely rare in children. Rare presentation, non-specific symptoms, insufficient experience, and characteristics mimicking other rectal diseases may cause misdiagnosis or delay of diagnosis in some pediatric patients. Here, we report a 10-year-old boy with solitary rectal ulcer diagnosed two years after onset of the symptoms who responded well to the conservative therapy, including high-fiber diet, laxatives, defecation training, and sucralfate enema.

    Rectum perforation after anorectal trauma in children: Report of two cases

    Anorectal injury is a rare clinical condition in children. Since it may be an initial sign of concomitant rectal and other intrapelvic structural injuries, this clinical condition is important to declare. Case 1: A nine-year-old boy was admitted to the hospital with complaints of abdominal pain and vomiting. Diffuse abdominal tenderness and defense were remarkable. Abdominal X-ray showed pneumoperitoneum. In operation rectum and bladder perforation were detected. Perforation was repaired and sigmoid colostomy was performed. Case 2: A six-year-old girl was admitted tot he hospital with perineal injury after a fall from height. A perineal wound which was 2 cm in diameter was detected at a localization 4 cm from the anal canal. Abdominal X-ray showed pneumoperitoneum. Rectal perforation was detected. Perforation was repaired and sigmoid colostomy was performed. To decrease the morbidity and the mortality of the patients, early and correct diagnosis, defining the type and extent of the injury including intrapelvic structures should be accurately determined and appropriate treatment should be performed. We think that colostomy should be done necessarily in cases when needed.

    Solunum yetmezliğine neden olan dev akciğer kist hidatiği : Olgu sunumu

    Hidatidozisin klinik bulguları kistin lokalizasyonu ve büyüklüğü ile ilişkilidir. Küçük kistler perforasyon veya enfeksiyon gibi komplikasyonlar gelişmediği sürece genellikle belirti vermezler. Ancak, büyük boyutlara ulaşmış kistler komşu organlara yaptığı bası ile semptomatik hale gelebilirler. Burada sol hemitoraksı dolduran dev hidatik kiste sahip ve solunum sıkıntısı nedeniyle acil cerrahi müdehale gerektiren 6 yaşındaki kız hasta sunulmuştur.
