Öğe Basal Septal Hypertrophy as the Early Imaging Biomarker for Adaptive Phase of Remodeling Prior to Heart Failure(Mdpi, 2022) Yalcin, Fatih; Yalcin, Hulya; Kucukler, Nagehan; Arslan, Serbay; Akkus, Oguz; Kurtul, Alparslan; Abraham, Maria RoselleHypertension plays a dominant role in the development of left ventricular (LV) remodeling and heart failure, in addition to being the main risk factor for coronary artery disease. In this review, we focus on the focal geometric and functional tissue aspects of the LV septal base, since basal septal hypertrophy (BSH), as the early imaging biomarker of LV remodeling due to hypertensive heart disease, is detected in cross-sectional clinic studies. In addition, the validation of BSH by animal studies using third generation microimaging and relevant clinical observations are also discussed in the report. Finally, an evaluation of both human and animal quantitative imaging studies and the importance of combined cardiac imaging methods and stress-induction in the separation of adaptive and maladaptive phases of the LV remodeling are pointed out. As a result, BSH, as the early imaging biomarker and quantitative follow-up of functional analysis in hypertension, could possibly contribute to early treatment in a timely fashion in the prevention of hypertensive disease progression to heart failure. Methods The study group consisted of 50 women; 25 patients each were given Tc-99m HIG or Tc-99m nanocolloid for lymphoscintigraphy. Then, intraoperative SLN localization with IGP was performed. The results of IGP, lymphoscintigraphy, blue dye injected just before surgery, and pathology were compared. Results In the Tc-99m HIG group, one patient had tumours in both breasts. In two patients, we could not detect SLNs with lymphoscintigraphy, although they were detected with IGP and blue dye. We found SLNs for all tumours with IGP. With the intraoperative blue dye, SLNs were identified for 25 tumours; for one tumour, no SLN was detected with blue dye. In the histopathological examination, 13 tumours showed metastasis in the SLN and in 11 of these 13, there were also metastases in the axilla. One patient had a skip metastasis. In the Tc-99m nanocolloid group, SLNs were identified in 24 patients with lymphoscintigraphy. IGP found SLNs in 24 patients. The blue dye detected SLNs in all patients. On histopathological examination, 10 patients had metastasis in the SLN and there were also metastases in the axilla in all of these patients. Conclusion Tc-99m HIG can be used in SLN detection with preoperative lymphoscintigraphy and IGP in early-stage breast cancer patients. In this study, our aim was to find the relationship between laboratory parameters and the maximum standard uptake value (SUVmax) of positron emission tomography/computed tomography (PET/CT) in patients with lymphoma at the diagnosis and after treatment. Methods: Thirty-four lymphoma patients treated at Mustafa Kemal University Internal Medicine Clinic between 2014 and 2017 were included in this retrospective study. Results of CRP, ESR, LDH, albumin, and white blood cell (WBC) count were recorded before each PET scan test, and each parameter was analyzed for correlation with SUVmax measurements. Results: Spearman's correlation test showed that the after-treatment SUVmax values were significantly correlated with the after-treatment LDH, ESR, and CRP values (for LDH, ESR, and CRP, R-2: 0.453, 0.426, and 0.351; P = 0.007, 0.012, and 0.042, respectively). On the other hand, albumin and WBC count did not show a significant correlation with the after-treatment SUVmax values (all P > 0.05). Conclusions: CRP, ESR, and LDH values may also be good predictors in patients for whom PET/CT imaging cannot be performed.Öğe The Evaluation of Solid Phase Gastric Emptying in Patients with Behcet's Disease(Derman Medical Publ, 2015) Dogramaci, Asena Cigdem; Yalcin, Hulya; Balci, Didem Didar; Ucar, Edip; Yenin, Julide Zehra; Kalender, EbuzerAim: Behcet's disease is a multisystemic disease that includes the mucocutaneous, ocular, cardiovascular, renal, pulmonary, joint and central nervous system involvement. Gastrointestinal system involvement is rare in Behcet's disease. Current study was planned to investigate the rate of gastric emptying in patients with Behcet's disease by using gastric emptying scintigraphy. Material and Method: In order to determine gastric emptying rate of solids, 14 patients with Behcet's disease and 14 healthy controls were studied scintigraphically. After an overnight fast, all subjects ingested a Tc-99m DTPA labeled solid meal consisted of an egg and toasted white bread. Immediately after ingestion of the meal, subjects were positioned supine under the gamma camera and serial images were recorded continuously for 90 minutes. Half empyting time (t 1/2) and percentage of radioactive material remaining in the stomach at 60 min. were calculated. Results: The mean ages of Behcet's patients was 41.00 +/- 10.25 years and 40.57 +/- 10.05 years in control group. T 1/2 of Behcet's patients was found 85 +/- 61 min. and 63 +/- 22 min. in control cases (p=0.122). Percentage of radioactive material remaining in the stomach at 60 min. was 69.3 +/- 14.0 % and 54.6 +/- 22.0 % in Behcet's patients and controls, respectively (p=0.035). Discussion: Although there was not a significant difference between half gastric emptying times, we determined that percentage of radioactive material remaining in the stomach at 60 min. was significantly greater in Behcet's patients than that in controls. As a result, gastric stasis could be seen in Behcet's patients due to multisystemic involvement.Öğe Graves' Disease with Exophtalmia in a Two-Year Old Child(Galenos Yayincilik, 2011) Yalcin, Hulya; Akcora, Bulent; Balci, AliThyrotoxicosis is one of the rare disorders diagnosed in childhood and adolescence. The most frequent cause is Graves disease. One of the Graves' disease complications is thyroid-associated orbitopathy. A 2-year-old girl was referred to our hospital for decreased weight gain. Her physical examination was normal except for a palpable thyroid tissue and exophtalmia. After laboratory examination, she was referred to the departments of nuclear medicine and radiology for the diagnosis of hyperthyroidism and Graves ophthalmia. When evaluated with the physical examination, laboratory and imaging results the patient was diagnosed as Graves' disease with orbital involvement. So the patient was taken on methimazole treatment.Öğe Overview of 99mTc-anti-TNF-? scintigraphy: Diagnostic applications(2014) Elboga, Umut; Kalender, Ebuzer; Yalcin, HulyaTumor necrosis factor alpha (TNF-?) has a role in the pathogenesis of several inflammatory diseases such as rheumatoid arthritis, Crohn's disease, and ulcerative colitis. As TNF-? plays a role in the pathogenesis of different inflammatory diseases, anti-TNF-? agents (monoclonal antibodies [mAbs]) such as infliximab, adalimumab, and certolizumab have been developed and investigated for the treatment of these conditions. In recent years, these mAbs also have been used for diagnosis, monitoring, follow-up of disease activity, and therapy decision-making for inflammatory diseases, especially rheumatoid arthritis, after labeling with 99mTc. However, 99mTc-anti-TNF-? imaging might have severe adverse effects and is expensive. In contrast, scintigraphic imaging before therapy with radiolabeled mAbs may be a cost-effective solution for therapy decisions. © 2014 Elboga et al.Öğe Prone imaging allows efficient radiopharmaceutical usage by obviating the necessity of a rest study in Tc-99m-methoxyisobutylisonitrile myocardial perfusion scintigraphy(Lippincott Williams & Wilkins, 2011) Gunay, Emel Ceylan; Erdogan, Alihan; Yalcin, Hulya; Kara, Pelin OzcanObjective In myocardial perfusion single-photon emission computed tomography studies, diaphragmatic attenuation of the inferior wall is a common artifact, which can be minimized by prone imaging. The aim of this retrospective study was to validate the impact of stress-prone imaging on the necessity of a rest perfusion study with a final goal of effective radiopharmaceutical usage. Methods The findings of 98 patients who had been examined by a combined supine and prone imaging were retrospectively evaluated. Prone acquisition was performed only when reduced perfusion was observed in the inferior wall on the stress supine images. The reconstructed images were evaluated both visually and quantitatively. Rest myocardial single-photon emission computed tomography study was omitted in patients with complete normalization of uptake in the prone images. Results obtained were also compared with the clinical data and follow-up. Results Prone imaging obviated the necessity of a rest perfusion study in 76 of 98 patients (77.5%). Among normally reported patients (n=89), 76 (85.4%) were saved from a rest study due to the prone study results. Prone study provided improved results in quantitative analysis as well. The difference between the mean summed stress scores of supine and prone studies was statistically significant in normally reported patients (P=0.013). However, no significant difference was observed in scores of ischemic patients (P=0.341). Conclusion Adding prone imaging to a post-stress Tc-99m-methoxyisobutylisonitrile myocardial perfusion study not only minimizes the inferior wall attenuation, but also reduces the need for a rest test, particularly in low-risk or intermediate-risk patients. Its etiopathogenesis and trigger factors still remain unclear. Parapsoriasis cases carry an increased risk of transformation into cutaneous T cell lymphoma. In the literature, there is no data about Helicobacter pylori ( H. pylori) frequency in parapsoriasis. This study aims to evaluate the relationship between H. pylori infection and parapsoriasis. Materials and Methods: The study included thirty patients with parapsoriasis who had no dispeptic complaint and thirty healthy individuals. The patient group was divided into two groups as patients with small plaque parapsoriasis (SPP) and those with large plaque parapsoriasis (LPP) based on clinical examination. Carbon 14 urea breath test was performed to evaluate presence of H. pylori in both patient and control groups. Results: Parapsoriasis cases had a significantly higher rate (18/30, 60.0%) of H. pylori positivity than controls (9/30, 30.0%) (p=0.020). Whereas patients with SPP and patients with LPP had similar rates of H. pylori positivity (55.6% vs 44.4%, p=0.279). Comparison of H. pylori status according to gender it was found that majority of H. pylori positive individuals were men, indicating a significant difference (p=0.014). Conclusion: Our results revealed that H. pylori infection is increased in patients with parapsoriasis. Therefore, it was thought that H. pylori may be associated with parapsoriasis and this agent may play a role in the etiopathogenesis of the diseaseÖğe Sensitivity of [18F]FDG PET/CT and classification of the primary tumor site in patients with carcinoma of unknown primary(Via Medica, 2022) Atilgan, Hasan Ikbal; Yalcin, HulyaBackground: The aim of this study is to find the sensitivity of the [F-18]FDG PET/CT and the classification of the primary sites of carcinoma of unknown primary (CUP) as a single-center experience. Material and methods: Sixty-eight patients with a mean age of 62.43 +/- 12.78 years were included in this study retrospectively. Sixty-five patients had biopsy or surgery after PET/CT, which revealed pathological diagnoses of malign primary tumors, while primary tumor site could not be detected in three patients with histopathological examination. We evaluated the primary site of CUP with [F-18]FDG PET/CT. Results: Primary sites of three patients were not determined by histopathological examination. Malign lesions indicating the primary site of tumor were identified in 52 of 68 patients with PET/CT correctly. The primary tumor was lung cancer in 14 patients, cholangiocellular cancer in 9 patients, lymphoma in 9 patients, pancreas cancer in 6 patients, gastric cancer in 4 patients, ovary cancer in 4 patients, colon cancer in 4 patients, breast cancer in 3 patients, hepatocellular cancer in 2 patients, rectal cancer in 2 patients, sarcoma in 2 patients, esophagus, renal cell cancer, squamous cell cancer, endometrium cancer, malign mela-noma, and multiple myeloma in 1 patient with histopathological examination. PET/CT was false positive in one patient. There were 13 patients in whom primary tumor could not be localized by PET/CT, but was diagnosed by histopathological evaluation. Conclusions: PET/CT should be the first-line diagnostic tool for CUP, other diagnostic imaging tools should be applied after a negative whole-body PET/CT.Öğe Sickle cell disease with regional silent cerebral infarction detected by SPECT(Lippincott Williams & Wilkins, 2007) Yalcin, Hulya; Melek, Ismet; Okuyucu, Esra; Reyhan, Mehmet; Akguel, Ferit; Serarslan, Gamze; Duman, TaskinAn 18-year-old boy was admitted to the hospital for a right leg ulcer, which appeared 1 year ago. After physical and hematologic examinations, he was referred for neurologic and cardiologic examination because his hemoglobin analysis and hematologic findings were interpreted as being consistent with sickle cell anemia. Although he had no neurologic symptoms, MRI and Tc-99m ethyl cysteinate dimer cerebral perfusion single photon emission computer tomography were performed to rule out a silent cerebral infarction. Changes secondary to an infarct were seen in the region of the caudate nucleus.Öğe Single photon emission computed tomography: An alternative imaging modality in left ventricular evaluation(Dove Medical Press Ltd, 2008) Yalcin, Hulya; Maza, Sofiane; Yalcin, FatihVarious diagnostic imaging modalities have been used for quantitative left ventricular (LV) parameters. Because of the suboptimal value of the most widely used technology, two-dimensional (2D) echocardiography, 3D ultrasonographic imaging has improved accuracy for LV volume and function. Single photon emission computed tomography (SPECT) is another diagnostic method where LV volumetric and functional parameters can be accurately provided by gated myocardial perfusion tomographic slices. First pass radionuclide venticulography is another imaging modality which has some practical limitations. Despite lower ejection fraction (EF) values compared with invasive approach, noninvasive techniques are accurate in determination of normal and depressed EF. Noninvasive techniques with 3D approach including gated SPECT are beneficial for not only global but also regional LV evaluation. It has been mentioned that the slight difference between echocardiography and SPECT could be caused by the diverse population studied. The results of diagnostic stress tests support that SPECT is feasible to use in evaluation of LV volume and functional analysis. Magnetic resonance imaging is an expensive modality to use routinely, but it preserves its importance in selected patients for providing precise LV geometric data.Öğe Stress-induced hypercontractility in patients with hypertension: An interesting imaging finding(Elsevier Ireland Ltd, 2010) Yalcin, Fatih; Yalcin, Hulya; Seyfeli, Ergun; Akgul, FeritExcessive sympathetic activity and stress-induced LV hypercontractility may be detected in hypertensives or hypertensive LV hypertrophy. Recent quantitative data support that hypertensive patients may be associated with stress-induced LV hypercontractility. Stress-induced hypercontractility may also be related to basal LV cavity obliteration and dynamic LV outflow tract obstruction in hypertensives. In contrast to LV cavity dilation, stress-induced LV cavity obliteration may be associated with favorable outcome, however it has not been completely elucidated. Hypertensives with dynamic LVOT obstruction may clinically present with acute heart failure and normal systolic function. LV contractility may change and evaluation of LV geometry, contractility and volume by imaging techniques may be important in disease progression. (C) 2009 Elsevier Ireland Ltd. All rights reserved.Öğe Thyroid Hemiagenesis With Subclinical Hyperthyroidism An Unusual Association(Lippincott Williams & Wilkins, 2010) Ozkan, Orhan Veli; Yonden, Zafer; Gokce, Cumali; Davran, Ramazan; Yalcin, Hulya; Savas, Nazan; Kaya, HasanA 21-year-old man was referred to our hospital for the evaluation and treatment of simple goiter. The patient's physical examination was normal except for a palpable left thyroid lobe (grade 1b). All the laboratory investigations and thyroid function tests were normal, except for a subnormal serum thyroid stimulating hormone level. The patient had no complaint of hyperthyroidism. Both the serum antibodies (antithyroglobulin and antithyroid peroxidase) were negative. Thyroid ultrasonography revealed the agenesis of the left lobe and isthmus of the thyroid. Thyroid scan with Tc-99 m pertechnate showed the absence of the left lobe and isthmus and diffusely enlarged right lobe hyperplasia. A follow-up of 6 months was carried out without any medication. Post follow-up, the thyroid function tests were still consistent with subclinical hyperthyroidism. In this report, we have described the case of a patient with thyroid hemiagenesis associated with subclinical hyperthyroidism and goiter.
