    Aberrant Internal Carotid Artery in the Tympanic Cavity

    Objectives:Presence of aberrant internal carotid artery (ICA) in the tympanic cavity is a very rare case and therefore the diagnosis is a challenge. Here, the authors aimed to address the importance of the diagnosis since the intervention to the middle ear and implants is increasing in number and this condition may lead to life-threatening bleeding.Methods:Between 2012 and 2016, among the patients referred to the authors' Radiology Department from the Department of Otolaringology for the evaluation of the temporal region for various indications, the authors reported this anomaly in 6 patients and multidetector computerized tomography of these 6 patients and magnetic resonance imaging of 2 were assessed in detail.Results:The mean age of the 6 patients was 28.8 and aberrant ICA abnormality was observed in 2 patients bilaterally and 4 patients unilaterally on the right side. In 1 patient ICA was not totally regressed and observed as hypoplastic. In one of the patients, dehiscence was evident between ICA and the cochlea. Moreover, in 1 patient the contralateral ICA was not observed.Conclusion:Although rare, aberrant ICA is an abnormality that should be kept in mind by the clinicians and the radiologists, since it leads to abundant bleeding when undiagnosed before the surgical interventions.

    Median arcuate ligament syndrome diagnosed with multisliced computerized tomography: A case report

    Median arcuate ligament syndrome occurs when median arcuate ligament pressures the root of the celiac artery and generally it reveals itself with the post-prandial pain. Because of the pressure there will be a significant tightness especially while expirium. Mesenteric ischaemia and low blood flow to gastro-intestinal system occurs related to the tightness and because of these we see the characteristic clinical symptoms of the syndrome. We present the patient whom we diagnosed with multislicde computerized tomograpy.

    Multislice bilgisayarlı tomografi ile median arkuat ligament sendromu tanısı : Olgu sunumu

    Median arkuat ligament sendromu, median arkuat ligamentin çöliak arter köküne basısı sonucu oluşur ve kendini genellikle postprandial ağrı ile belli eder. Basıya bağlı, özelikle ekspiryumda daha belirgin darlık oluşur. Darlığa bağlı mezenterik iskemi ve gastro-intestinal sisteme yetersiz kan akımı olması nedeniyle sendroma ait karakteristik klinik bulgular görülür. Biz kliniğimize gelen ve multislice bilgisayarlı tomografi ile tanısı konan hastayı sunuyoruz.

    Osteolitik benign kalvarial kitlelere radyolojik yaklaşım

    Kalvarial lezyonlar histopatolojik olarak, kongenital, enflamatuar, neoplastik ve travmatik kökenli olup, nadir olarak izlenmektedir. Kalvarial lezyonlar en sık klinik muayenede ele gelen kitle şeklinde ya da radyolojik bir tetkikde insidental olarak tanı konur. Kemik yapıdaki defekt, lizis ve skleroz radyografi ve bilgisayarlı tomografi (BT) ile, lezyonların yumuşak doku komponentleri ve çevre yumuşak doku ile ilişkisi, BT ve manyetik rezonans görüntüleme (MRG) ile ve özelikle MRG ile değerlendirilir. Radyolojik değerlendirme sonucu, radyolojik bulgulara göre kalvarial lezyonların benignmalign ayrımı ve histopatolojik tanısını tahmin etmek mümkün olmaktadır.

    The prevalence of silent cerebral infarct in patients with sickle cell disease

    Objective: To determine the prevalence of silent cerebral infarct and abnormal brain magnetic resonance imaging (MRI) findings in patients with sickle cell disease (SCD). Materials and methods: Fifty-two brain MR images of the asymptomatic patients with SCD have been reviewed retrospectively. The abnormal MRI findings were recorded. Results: Forty-four of the 52 patients with SCD were homozygous (S/S) and remaining 8 heterozygous (S/ßthal) for SCD. Twelve (27.2%) of the 44 patients with S/S and 2 (25%) of the 8 patients with S/ßthal had silent infarcts. Ten (22.7%; 7 diffuse cerebral, 2 focal cerebral and 1 cerebellum) of the 44 S/S patients demonstrated atrophy. None of the 8 patients with S/ßthal had cerebral atrophy. Conclusion: Patients with SCD living in Hatay demonstrated a high rate of silent cerebral infarct and cerebral atrophy. These findings concur with the results of the literature.

    Radiological approach to osteolytic benign calvarial lesions

    Histopathologically, calvarial lesions are congenital, inflammatory, neoplastic and traumatic origin and these are rarely seen. The calvarial lesions most commonly manifest as palpable mass on physical examination or they are diagnosed incidentally during a radiological test. Defect, lysis and sclerosis in the bony structure are evaluated with radiography and computed tomography (CT) and relationships of the lesions with the soft tissue components and surrounding soft tissue are evaluated with CT and magnetic resonance imaging (MRI) and especially with MRI. Based on the radiological findings, benign-malignant discrimination of the calvarial lesions can be made and it may be possible to estimate their histopathological diagnoses.

    Radiological Imaging of Rare Intracranial Complications Secondary to Otitis Media and Mastoiditis

    Acute otitis media used to cause fatal results because of its intracranial complications before the introduction of potent and effective antibiotics. After the introduction of antibiotics, complications have started to be observed more frequently as a result of chronic otitis media and especially in children. Because clinical findings changed and became indistinct, the diagnosis of otitis and mastoiditis has been made occasionally with imaging findings only after complications occurred. Multidetector computed tomography and magnetic resonance imaging are efficient and sufficient methods in the rapid diagnosis and should be immediately referred methods.

    Two cases with giant pseudo-aneurysm of vertebral artery secondary to shrapnel injury

    Due to their locations, vertebral arteries (VA) are structures that are rarely injured after traumas. Formation of vertebral pseudo-aneurysm (PA) because of pieces of shrapnel is also rare. It causes clinical findings like cervical mass after the injury Two Syrian 12- and 17-year-old male patients were admitted to the Department of Ear, Nose and Throat with the complaint of swelling in the left lower cervical region. In their USG examination which was ordered to investigate the cervical lesion, a lesion consistent with PA which shows a prominently thick wall structure due to circumferential thrombus formation and fibrotic tissue, chaotic flow, and ying-yang appearance in the centrally located cavity of the lesion in a color Doppler examination was detected. Later, multi-detector computed tomography angiography (MDCTA) was planned to demonstrate pseudo-aneurysms (PAs). Because there was no report on the development of giant pseudo-aneurysms secondary to shrapnel injury to date and because we thought these cases are demonstrative, we found the cases suitable for presenting as case reports.
