    Benign prostat hiperplazili hastalarda alfa-1 adrenerjik reseptör blokerlerin alt üriner sistem semptomları ve cinsel fonksiyonlar üzerine etkileri

    Amaç:Bu çalışmada Benign Prostat Hiperplazili (BPH) hastalarda alfa-1 adrenerjik reseptör blokerlerinin alt üriner sistem semptomları (AÜSS) ve cinsel fonksiyonlar üzerine etkilerini değerlendirmek amaçlanmıştır.Yöntem: Mart 2012 ile Eylül 2012 tarihleri arasında Mustafa Kemal Üniversitesi Eğitim ve Araştırma Hastanesi Üroloji polikliniğine başvuran, BPH nedeniyle medikal tedavi alması gereken 40-80 yaş arası 78 hasta çalışmaya alındı. Randomize olarak 3 gruba ayrıldı. T grubu günlük 0,4 mg tamsulosin, D grubu günlük 4mg doksazosin, A grubu 10 mg alfuzosin aldılar. Hastalara uluslararası prostat semptom skoru (IPSS), uluslaraarsı cinsel işlev indeksi-5 (IIEF-5), üriner semptomlara göre yaşam kalitesi ölçeği (QoL) ve ejekülasyon değerlendirme anketi yapıldı. Bu anketler tedavi başlangıcında, 4 hafta sonra ve 12 hafta sonra yinelendi. Hastaların üroflowmetri parametreleri, idrar yapımı sonrası kalan miktar ölçümü (PVR) başlangıçta, tedavi başlangıcından 4 hafta sonra ve 12 hafta sonra yapıldı.Bulgular: Her üç grupta tedaviden 4 hafta ve 12 hafta sonra IPSS skorunda, QoL ölçeğinde, PVR değerinde düşme, Qmax değerinde yükselme gözlenmiş ve gruplar arası anlamlı fark bulunamadı (p<0,001). IIEF-5 değeri 4 hafta sonra T grubunda azalmış, D grubunda ise artmıştır (p<0,001). A grubunda ise 14,50±6,58'den 13,61±6,28'e düşüş gözlenmiş ancak anlamlı bulunamadı (p=0,12). 12 hafta sonunda T grubunun % 52,1'inde, D grubunun %9,5'inde ve A grubunun %22,7'sinde anormal ejekülasyon vardı. Ancak tedavi başlamadan önce yapılan sorgulamada anormal ejekülasyon D grubunda %10, A grubunda %28,5 ve T grubunda %16,6'ydı.Sonuç: Bu üç alfa blokerin AÜSS'na pozitif etkileri olmakla birlikte gerek ejekülasyon üzerine gerekse ereksiyon gibi cinsel fonksiyonlar üzerinde etkilerini netleştirmek için daha fazla hasta sayısına ve izlem süresine gereksinim vardır.Anahtar Sözcükler: Alt Üriner Sistem Semptomları, Prostat, Benign Prostat Hiperplazisi

    The effect of tyrosol on diclofenac sodium-induced acute nephrotoxicity in rats

    Although diclofenac (DCF) is a nonsteroidal anti-inflammatory drug that is considered safe, its chronic use and overdose may show some toxic effects. The protective effect of tyrosol (Tyr) pretreatment against DCF-induced renal damage was investigated in this study. The 32 rats used in the study were randomly divided into four groups of eight rats each. According to the data obtained, it was determined that creatinine, urea, and blood urea nitrogen (BUN) levels increased in serum samples of the DCF group. Besides, the levels of reduced glutathione (GSH) and glutathione peroxidase (GPx) activity decreased and the malondialdehyde (MDA) level increased in the kidney tissue. However, no change was observed in catalase (CAT) activity. Cyclooxygenase-2 (COX-2), nuclear factor kappa B (NF-kappa B), and tumor necrosis factor-alpha (Tnf-alpha) levels increased and nuclear factor erythroid 2-related factor 2 (Nrf-2) levels decreased. No change was detected in the level of interleukin 1 beta (IL-1 beta). When the DCF+Tyr group and the DCF group were compared, it was assessed that Tyr had a curative effect on all biochemical parameters. Also, kidney damages, such as degeneration and necrosis of tubular epithelium and congestion of veins, were obviated by treatment with tyrosol in histopathological examinations. It was determined that Tyr pretreatment provided a protective effect against nephrotoxicity induced by DCF with its anti-inflammatory and antioxidant properties.

    Hatay ili 7-11 yaş arası dış genital organ anomali oranları

    Amaç: Bu çalışmada Hatay il merkezinde öğrenim gören ilköğretim çağındaki erkek çocuklarda dış genitaorgan bozukluğu sıklığı ve çeşitliliğinin belirlenmesi hedeflendi. Yöntem: Mustafa Kemal Üniversitesi Etik komitesi tarafından çalışma onaylandıktan sonra, il merkezindek6 adet ilköğretim okulunda öğrenim görmekte olan ve rastgele örnekleme yöntemi ile 7-11 yaş arasındaktoplam 981 erkek öğrenci çalışmaya dahil edildi. Tüm öğrenciler iki üroloji uzmanı tarafından okullarında muayene edildi ve tespit edilen dış genital organ anomalileri kaydedildi. Bulgular: Çalışmaya katılan 981 öğrencinin 126’sında (%12,8) anomali saptandı. Saptanan anomalilerinmemiş testis 60(%6,1),retraktil testis 49 (%5), inguinal herni 7(%0,7), hipospadiyas 4(%0,4), hidrose4(%0,4) ve varikosel 2(%0,2) idi. Çalışmamızda, Hatay il merkezinde ilköğretim çağındaki çocuklardagenital anomali oranını tespit etmeye çalıştık. Sonuç: Dış genital anomali oranları diğer yapılan tarama çalışmalarında birbirine yakın değerler bulunsa bile ülkemizde dış bölgelere göre farklılıklar izlenmektedir.

    Hereditary Behavior of Varicocele

    The inheritance of varicoceles and the potential transmission to first-degree relatives has rarely been investigated. In the present study, we examined the first-degree relatives of men with known varicocele to reveal the familial risk for varicocele. Of the patients with clinical varicocele who presented with infertility, testicular pain, or asymmetrical swelling of the scrotum between June 1, 2008 and May 31, 2009, 49 agreed to have their available first-degree relatives contacted for screening of varicoceles (n = 66). A cohort of 100 consecutive men who applied to the department of internal medicine between 2008 and 2009 for checkup procedure without a history of subfertility or a varicocele were used as a control population. Of the 92 first-degree relatives contacted, 66 (71.7%) decided to participate in this study. Of these 66 men, 21 (33.9%) had a palpable varicocele on physical examination. Compared with a control population (12%), the prevalence of palpable varicocele in the first-degree relatives of patients with known varicocele (33.9%) was approximately 3-fold greater (P < .005). Among the first-degree relatives, 4 (21.1%) of 19 fathers and 17 (36.2%) of 47 brothers had palpable varicocele. As a conclusion, a significant increase in varicocele prevalence is present in the first-degree relatives of men with known varicoceles. Patients should be counseled about this increased risk in male relatives of patients.

    Improvement of monosymptomatic enuresis after adenotonsillectomy in children with obstructive sleep apnea syndrome

    Aim: To investigate the prevalence of monosymptomatic enuresis (ME) in children diagnosed with obstructive sleep apnea syndrome (OSAS) and the rate of resolution or improvement in enuresis following adenotonsillectomy. Materials and methods: We retrospectively reviewed the charts of 541 consecutive patients who underwent adenotonsillectomy for OSAS secondary to adenotonsillar hypertrophy between January 2005 and January 2009. All of the charts for patients between 5 and 18 years of age at the time of surgery (n = 398) were included in the study. After reviewing the charts, the families were contacted by telephone. Those patients who had shown preoperative symptoms of enuresis were questioned to determine whether there had been any change in their complaints postoperatively. Results: Of the 398 patients whose records were reviewed, 98 were excluded from this study because of incomplete records. The incidence of ME in the study group (n = 300) prior to adenotonsillectomy was 30.7% (92 patients). Among these 92 patients, 64 (69.6%) were male and 28 (30.4%) were female (P = 0.001). The parents of 46 of these 92 patients agreed to allow their children to participate in the study. In 46 patients, 26 (56.5%) had complete resolution, 8 (17.4%) had a partial improvement, and 12 (26.1%) had no change in enuresis following adenotonsillectomy. Patients with OSAS had a 2.38-fold higher risk of ME (odds ratio 2.38, 95% confi dence interval 1.60 to 3.53, P = 0.001). Conclusion: Children with OSA symptoms have a high rate of monosymptomatic enuresis. Relief of OSA symptoms also resulted in the complete resolution or partial improvement of ME in more than two-thirds of patients. In the diff erential diagnosis of a child presenting with enuresis, OSAS should be kept in mind and, conversely, the presence of enuresis should be investigated in children presenting with OSA symptoms.

    Molecular epidemiology and risk factors in extended-spectrum beta lactamase positive and quinolone-resistant Escherichia coli strains isolated from urinary tract infections

    Introduction: The study was designed to investigate the epidemiology, risk factors, and clonal relationships in extended-spectrum beta lactamase (ESBL)-positive and quinolone resistant Escherichia coli strains isolated from urinary tract infections. Materials and methods: The study included 96 E. coli strains isolated from urine samples and identified as the infection agent. All the strains were ESBL positive and quinolone resistant. ESBL production was evaluated by the double disc synergy test. The clonal proximity of the isolates was determined with Rep-PCR method. Results: Fifty-two (54.2%) of the 96 isolates were isolated from nosocomial infections, and 44 (45.8%) were isolated from community- acquired infections. Three common risk factors for ESBL-positive E.coli infections were identified. The most common risk factor was hospitalization in the last 6 months. The Rep-PCR technique detected 29 clones, of which nine were major clones and 20 were sporadic. Conclusion: The identification of clonal relationships using Rep-PCR can reveal the source of infections and means of dissemination. Community-acquired isolates that cause infections may be associated with hospitalization. Strains colonized in hospital for along time can increase infections in various departments. An increase in the resistance to any drug in any unit of a hospital may be the harbinger of a pandemic. The source of infection and ways of spread can be identified most accurately by the demonstration of clonal relationships.

    A new anthropometric measurement of penile length and its relation to second and fourth digital lengths

    Amaç: Ürogenital sistem farklılaşmasını kontrol eden Hox genleri parmakların gelişimini de kontrol etmektedir. Bu çalışmada penis uzunluğu ile ikinci ve dördüncü parmakaların uzunlukları arasındaki ilişki değerlendirildi. Yöntem ve gereç: Bu çalışmaya doğu akdeniz bölgesindeki ilkokul çağındaki toplam 1028 Türk erkek çocuğu dahil edilmiştir. Vücut kitle indeksini hesap edebilmek için ağırlık ve uzunlukları ölçülmüştür. Penis uzunlukları gevşek ve gerilmiş halde ölçülmüştür. Sağ el ikinci ve dördüncü parmakların uzunlukları ölçülmüştür. İstatiksel ölçümler deskriptif istatistik ve Pearson korrelasyon analizi ile yapılmıştır. Bulgular: Aşağıdaki karşılaştırmalar arasında anlamlı korelasyon tespit edilmiştir: ikinci parmak uzunluğu ile gevşek ve gerilmiş penis uzunluğu (r = 0,166 and r = 0,276, sırasıyla; P < 0,01); dördüncü parmak uzunluğu ile gevşek ve gerilmiş penis uzunluğu (r = 0,174 and r = 0,287, sırasıyla; P < 0,01). 2D:4D oranı ile gevşek ve gerilmiş penis uzunluğu arasında anlamlı korelasyon bulunmamıştır (r = –0,013; P = 0,668, r = –0,018; P = 0,565). Sonuç: Sonuç olarak, çalışmamızdaki pozitif korelasonlar yeni bir bulgudur ve içerisinde daha başka klinik anlamlar içerebilir.

    Protective Effects of Colchicine on Testicular Torsion/Detorsion-Induced Ischemia/Reperfusion Injury in Rats

    Purpose: To evaluate the short-term use of colchicine on preventing ischemia-reperfusion injury after surgery in an experimental animal model. Materials and Methods: A total of 40 rats were divided into five groups (n = 8). Sham (Sh), ischemia-reperfusion (I/R), I/R and colchicine-treated for once per-operatively (I/Rcl), I/R and colchicine-treated for 5 days postoperatively (I/Rc5), and I/R and placebo given for 5 days (I/Rp) groups. Testicular torsion was created by rotating the testicle 720o in clockwise direction and held for 3 hours. In group I/Rcl 30 minutes before detorsion, p.o. 1 mg/kg mL infusion of colchicine was given only once. In group I/Rc5, colchicine continued p.o. once daily for five days. Tissue malonyldialdehite (MDA), superoxide dismutase (SOD), glutathione peroxidase (GPx) and catalase (CAT) were measured for evaluating the oxidative stress. Apoptosis levels shown with Caspase-3 staining and mean seminiferous tubular diameter (MSTD), germinal epithelial cell thickness (GECT), and mean testicular biopsy score (MTBS) were used to evaluate the germ cell damage. Results: Decreased protein MDA levels therewithal increased SOD, CAT and GPx levels achieved in I/Rc5 group when compared to I/R group and did not differ from the I/Rp group (p < 0.05). MSTD, GECT, and JS were better in I/Rc5 than I/Rp which showed the natural course of I/R damage in testis (p < 0.005). Caspase 3 positivity, as an apoptosis indicator, were significantly lower (p < 0.05) in I/Rc5 group in comparison with I/R, I/Rcl, and I/Rp groups. Conclusion: The usage of colchicine as a complementary treatment after definitive surgery reduce early-onset ischemia-reperfusion damage and diminishes apoptosis.
