Psöriazis hastalarında serum elafin düzeyleri ile hastalık arasındaki ilişkinin saptanması, sedimentasyon ve CRP ile elafin düzeyleri arasındaki korelasyonun araştırılması

Özet
Amaç: Psöriazis hastalarında serum elafin düzeylerinin psöriazis hastalığındaki rolünü incelemek, CRP ve sedimentasyon düzeyleri ile serum elafin düzeyleri arasındaki ilişkiyi belirlemektir. Materyal ve Metod: Çalışmanın örneklemi, Hatay Mustafa Kemal Üniversitesi Hastanesi Dermatoloji polikliniğinde psöriazis tanısı alan 40 hastadan ve hasta grubu ile aynı özelliklere sahip 40 sağlıklı bireyden oluşmaktadır. Tüm bireylerin demografik bilgileri, vücut kitle indeksi, bel çevresi ve kan basıncı değerleri kaydedildi. Hasta grubunda psöriazis alan şiddeti indeksi (PAŞİ) hesaplandı. Her iki çalışma grubunun serumunda, elafin, C-reaktif protein (CRP), sedimentasyon, açlık kan şekeri, total kolesterol, trigliserit, HDL (yüksek dansiteli lipoprotein ve LDL (düşük dansiteli lipoprotein) düzeyleri ölçüldü. Bulgular: Çalışmamızda hasta ve kontrol grubu arasında serum elafin, HDL ve LDL düzeyleri açısından istatistiksel olarak anlamlı bir farklılık tespit edildi. Hasta grubunda serum elafin ortanca değeri 306,44 pg/mL, kontrol grubunda serum elafin ortanca değeri ise 222,33 pg/mL idi (p=0,028). Hasta grubunda HDL ortanca değeri 47,5 mg/dL, kontrol grubunda HDL ortanca değeri 55 mg/dL idi (p=0,004). Hasta grubunda LDL ortanca değeri 89 mg/dL, kontrol grubunda LDL ortanca değeri 80 mg/dL idi (p<0,001). Hasta grubunda serum elafin düzeyi ile LDL (p<0,001) ve total kolesterol (p<0,001) arasında pozitif yönde güçlü bir ilişki tespit edildi. Yapılan ROC analizi neticesinde psöriazis hastalarının tanısında serum elafin düzeyinin başarılı bir performansa sahip olduğu tespit edildi (p=0,028). Sonuç: Hasta ve kontrol grubu arasında serum elafin, HDL ve LDL düzeyleri açısından anlamlı fark tespit edildi. Sonuç olarak elafin, psöriazis patogenezinde önemli bir role sahiptir ve bu sonuçların doğru kullanımı, hastalığın ilerleyişini, şiddetini ve gelecekteki potansiyel terapötik çizgileri değerlendirmede faydalı olacağı düşüncesindeyiz.
Aims: The aim of this study is to examine the role of serum elafin levels in psoriasis patients, and to determine the relationship between CRP and sedimentation levels and serum elafin levels. Demographic information, body mass index, waist circumference and blood pressure values of all individuals participating in the study were recorded. The psoriasis area severity index (PASI) was calculated in the patient group. Elafin, C-reactive protein (CRP), sedimentation, fasting blood sugar, total cholesterol, triglyceride, HDL (high-density lipoprotein and LDL (low-density lipoprotein) levels were measured in the serum of both study groups. Results: In our study, a statistically significant difference was found between the patient and control groups in terms of serum elafin, HDL and LDL levels. The median serum elafin value in the patient group was 306,44 pg/mL, the median serum elafin value in the control group was 222,33 pg/mL (p=0,028). The median HDL value in the patient group was 47,5 mg/dL, the median HDL value in the control group was 55 mg/dL (p=0,004). The median LDL value in the patient group was 89 mg/dL, the median LDL value in the control group was 80 mg/dL (p<0,001). A strong positive correlation was found between serum elafin level and LDL (p<0,001) and total cholesterol (p<0,001) in the patient group.
