Özet
Amaç: Çocuk yoğun bakım ünitesine (ÇYBÜ) yatırılan hastaların tanısı, cinsiyeti, yatış süresi, hangi aylarda yattığı ve sonlanımı incelendi. Yöntem: Bu çalışmada Kasım 2002 - Şubat 2005 tarihleri arasında Dicle Üniversitesi Tıp Fakültesi Çocuk Sağlığı ve Hastalıkları Çocuk Yoğun Bakım Ünitesine yatırılarak takip edilen 698 hasta değerlendirildi. Bulgular: Hastaların 95’i(%13.6) akciğer enfeksiyonu, 84’ü(%12) sepsis, 70’i(%10) zehirlenme, 49’u(%7) merkezi sinir sistemi enfeksiyonu, 44’ü(%6.3) status epileptikus, 28’i(%4) hepatik koma, 27’si(%3.9) yılan ısırması, 24’ü(%3.4) akrep sokması, 18’i(%2.6) travma, 17’si(%2.4) intrakranyal kanama, 15’i(%2.1) meningokoksemi, 12’si(%1.7) suda boğulma, 8’i(%1.14) elektrik çarpması, 7’si(%1) malignensi tanıları ile yatmıştı. Ocak (%10.7) ve Haziran (%10.4) aylarında daha fazla yatış görülürken, en az yatış Ekim (%5.7) ayında gözlendi. Kız hasta sayısı 300(%42.8), erkek hasta sayısı 398(%57.2) idi. Yatış süreleri ortalama 5.3±6.1 gün idi. Hastaların 363’ü(%51.8) durumu stabil edildikten sonra diğer servislere transfer edildi, 86’sı(%12.3) yoğun bakım ünitesinden taburcu edildi, 8’i(%1.14) ileri merkeze sevk edildi, 238’i(%34.4) ise öldü. Sonuç: Bu çalışmada akciğer enfeksiyonu ve bu nedenle solunum desteğinin yoğun bakım ünitesinde en sık karşılaşılan durum olduğu, çocukluk çağı zehirlenmeleri, travma, suda boğulma, elektrik çarpması gibi önlem alınabilecek hastalıkların yatışlarda önemli bir yer tuttuğu vurgulanmak istendi.
Objective: The patients hospitalized in Pediatric Intensive Care Unit (PICU) were evaluated for the diagnosis, gender, length of stay, months at which they were hospitalized and discharged. Method: In this study, 698 patients hospitalized and followed-up in the Pediatric Intensive Care Unit of Pediatric Health and Diseases of Dicle University Faculty of Medicine between dates of November 2002 – February 2005 were evaluated. Results: 95 (13.6%) of patients were hospitalized with the diagnosis of pulmonary infection, 84 of those (%12) with sepsis, 70 (10%) with poisoning, 49 (7%) with infection of central nerve system, 44 (6.3%) with status epilepticus, 28 (4%) with hepatic coma, 27 (3.9%) with snakebite, 24 (3.4%) with scorpion bite, 18 (2.6%) with trauma, 17 (2.4%) with intracranial bleeding, 15 (2.1%) with meningococcemia, 12 (1.7%) with drowning, 8 (1.14%) with electric shock, and 7 of patients (1%) with malignancy. While most frequent hospitalization was recorded in January (10.7%) and June (10.4%), the least frequent hospitalization was observed in October (5.7%). Number of female patients was 300 (42.8%), whereas number of male patients was 398 (57.2%). Mean length of stay was 5.3±6.1 days. 363 (51.8%) of patients were transferred to other services after their general condition had been stabilized, 86 (12.3%) of patients were discharged from the intensive care unit, 8 (1.14%) of patients were referred to an advanced center, and 238 (34.4%) of patients died. Conclusion: In this study, it is intended to highlight the fact that the pulmonary infection and thus the respiratory support are the most frequently recorded conditions in the intensive care unit and that preventable diseases such as poisoning in childhood, trauma, drowning, and electric shock take an important place in hospitalization.
