Özet
Adli olguları içerisinde ateşli silah yaralanmaları önemli yer tutmaktadır. Ancak bu tür yaralanmalara bağlı olfaktor sinir lezyonu nadir görülmektedir. Bu çalışmada av tüfeği saçma tanesi yaralanması sonucu gelişen olfaktor sinir kaybı olan bir olgunun sunulması amaçlandı. Olgumuz erkek ve 31 yaşındadır. Yapılan muayenesinde baş, boyun ve göğüs bölgesinde av tüfe- ği saçma tanelerine ait yaralanmaları mevcuttu. BT'sinde sol ön fossada saçma tanesi oldu- ğu tespit edilmişti. Tedavisinin bitiminden sonra iyileşen hastanın koku duyusunun olmadı- ğını ifade etmiş. Bunun üzerine savcılık tarafından yeniden adli rapor düzenlenmesi istenmişti. Yapılan KKB muayenesinde ve yapılan tetkikler sonucunda olfaktor sinirin yaralandığını, 20 çeşit koku testinin negatif sonuç verdiğini ve buna bağlı koku duyusunun tamamen kaybedildiği saptandı. Düzenlenen adli raporda koku duyusunun kaybı "herhangi bir duyu ve organda işlev yitirilmesi" niteliğinde olduğu kararına varıldı. Yaralanma sonucu oluşan lezyon ilginç bulunduğundan sunulmuştur.
Firearm wounds are one of the main topics in forensic medicine. However, olfactory nerve lesion due to these types of injuries is rarely seen. In this study, we aimed to report a case with loss of olfactory nerve due to being injured by pellet of shotgun. Our case was a 31-year old male. There were damages due to pellets of shotgun in head, neck and chest regions in physical examination. Presence of a pellet in the left anterior fossa was determined on a CT scan. After the patient recovered, he reported that he couldn’t smell. Thereupon, judicial report was asked to be rearranged by prosecution office. It was determined that olfactory nerve had been injured. 20 types of smell tests gave negative results. O t o r h i n o l a r y n g o l o g i c a l examination revealed that sense smell was completely lost. In forensic report, it was concluded that the loss of the smelling sense had the characteristics of “loss of function in any sense and organ”.
