Kedilerde medyan hat ve flank laparotomik ovaryohisterektomi operasyonlarının karşılaştırılması

Özet
Sunulan çalışmada sağlıklı dişi kedilerde elektif ovaryohisterektomi (OHE) uygulanmasında flank laparotomik ve geleneksel medyan hat yaklaşımının operatif ve postoperatif dönemdeki etkilerinin karşılaştırılması amaçlandı. Çalışmada kısırlaştırma operasyonu talebiyle hastanemize gelen kedilerden medyan OHE (MOHE, n=17) ve sol flank yaklaşım (FOHE, n=15) grupları oluşturuldu. Gruplarda intraoperatif dönemde operasyon süresi, ensizyon hattı uzunluğu, kanama ve doku rupturu komplikasyonları ile deri sutur adetleri takip edildi. Erken postoperatif dönemde iyileşmenin takibinde çeşitli değişkenler (Agresyon, ajitasyon, gönüllü gıda alımının başlaması, su tüketiminin başlaması, tam uyanma süresi, düzgün yürüme zamanı, mama ve kum kabına gitme davranışları) takip edildi. Bu değişkenler süresine göre skorlanarak not edildi. Postoperatif 3. günde tüm kedilerin pansumanı değiştirildi ve yara hattında ağrı/hassasiyet, eritem, ödem, akıntı varlığı ile kedilerin yara hattını yalama davranışı takip edildi. Tüm kedilerden operasyon öncesi (0. gün/0. saat), postoperatif 2. saat, postoperatif 3. gün ve postoperatif 10. gün kan numuneleri alındı. Cerrahi stres yanıtının değerlendirilmesi amacıyla operasyon öncesi ve postoperatif 2. saatte alınan kanların serumlarında IL-6 konsantrasyonu ELISA yöntemi ile ölçüldü. Cerrahi yaklaşımlar arasında kas hasarı ve kas iyileşmesinin değerlendirilmesi amacıyla operasyon öncesi, postoperatif 3. gün ve postoperatif 10. gün kan serumlarında LDH, AST ve CK enzim seviyeleri oto-kimya analizör cihazı yardımıyla belirlendi. FOHE grubunda ensizyon hattı uzunluğu MOHE grubundan anlamlı derecede kısaydı (P<0,05). Takip edilen diğer parametreler gruplar arasında anlamlı derecede farklı değildi (P>0,05). Kas iyileşmesinin takibinde değerlendirilen enzimlerden CK, gruplar arasında anlamlı derecede değişmedi (P>0,05), ancak her bir grubun örneklem günleri arasındaki değişim anlamlı derecede farklıydı (P<0,05). Sonuç olarak flank laparatomik ve medyan hattan gerçekleştirilen ovaryohisterektomi operasyonlarının operatif komplikasyonlar, cerrahi stres yanıtı, postoperatif iyileşme süreci ve postoperatif komplikasyonlar açısından farklı olmadığı görüldü. Yöntemlerin birbirine üstünlüğü bulunmaması nedeniyle elektif kısırlaştırma operasyonlarında seçilecek yaklaşım hastanın durumundansa hekimin tercihine göre şekillenebileceği kanısına varıldı
The aim of the presented study was to compare the operative and postoperative effects of flank laparotomy and traditional median line approach in elective ovariohysterectomy (OHE) in healthy female cats. In the study, median OHE (MOHE, n=17) and left flank approach (FOHE, n=15) groups were formed from cats brought to our hospital with a request for spaying surgery. During the intraoperative period, operation time, incision length, complications of bleeding and tissue rupture, and number of skin stitches were monitored in the groups. Various variables (aggression, agitation, onset of voluntary food intake, onset of water consumption, time to fully wake up, time to walk properly, behavior of going to the food and litter box) were monitored to monitor recovery in the early postoperative period. These variables were noted by scoring according to duration. On the 3rd postoperative day, the dressings of all cats were changed and the presence of pain/tenderness, erythema, edema, discharge in the wound and the cats' licking behavior on the wound line were monitored. Blood samples were taken from all cats before the surgery (0th day/0th hour), at the 2nd postoperative hour, on the 3rd postoperative day and on the 10th postoperative day. In order to evaluate surgical stress, IL-6 concentration in blood serum taken before the surgery and at the 2nd postoperative hour was measured by ELISA method. In order to evaluate muscle damage and muscle recovery among surgical approaches, LDH, AST and CK enzyme levels were determined in blood serum that taken before the surgery, on the 3rd postoperative day and on the 10th postoperative day, with an auto-chemistry analyzer device. The length of the incision in the FOHE group was significantly shorter than in the MOHE group (P<0.05). Other parameters monitored were not significantly different between groups (P>0.05). CK, one of the enzymes evaluated in the follow-up of muscle recovery, did not change significantly between the groups (P>0.05), but the change between the sampling days of each group was significantly different (P<0.05). As a result, it was observed that flank laparotomy and median line ovariohysterectomy operations were not different in terms of operative complications, surgical stress response, postoperative recovery process and postoperative complications. Since the methods do not have any superiority over each other, it was concluded that the approach to be chosen in elective sterilization surgery can be shaped according to the surgeon's preference rather than patience condition.
