Raloksifenin postmenopozal hastalarda ürogenital septomlar ve cinsel fonksiyon üzerine etkisinin değerlendirilmesi

dc.description.abstractAmaç: Herhangi bir topikal östrojen tedavi almayan hastalarda, raloksifenin ürogenital semptomlar ve cinsel fonksiyon üzerine etkisini araştırmak Gereç ve Yöntemler: En az üç yıl süreli amenoresi olan ve hormon replasman tedavisi kullanmak istemeyen 44 hastaya altı ay boyunca raloksifen verilirken kontrol grubu olarak raloksifen kullanmayan 46 hasta seçildi. Cinsel istek, ilişki sıklığı, cinsel tatmin, vajinal kuruluk ve disparoni ile ilgili bilgileri değerlendirilerek bir cinsel fonksiyon skoru belirlendi. Vajinal rugaların durumu, mukoza rengi, peteşi varlığı ve elastisiteye göre vajinal atrofi skoru belirlendi. Urge ve stres inkontinans, inkontinans sıklığı ve şiddeti esasen hikaye, pelvik muayene ve öksürük stres testi ile değerlendirildi. Bulgular: Hastaların 6 aylık tedavi sonrası değerleri bazal değerlerle karşılaştırıldığında cinsel fonksiyon skorunun bazal değerlerden anlamlı ölçüde düştüğü gözlendi (P = 0.002) ancak 6 aylık tedavi sonrası azalma miktarı gruplar arasında farklılık göstermemekteydi (P > 0.05). Vajinal atrofi skorunda bir artış saptansa da bu artış istatistiksel olarak anlamlı değildi ve gruplar arasında da bir fark saptanamadı (P > 0.05). İnkontinans tipleri, sıklıkları ve şiddeti açısından da bazal değerlerden ve gruplar arasında anlamlı bir farklılık saptanmadı (P > 0.05). Sonuç: Bu çalışmada, raloksifenin cinsel fonksiyon, vajinal mukoza ve pelvik taban yapısı üzerinde olumsuz bir etkisinin olmadığı gözlendi. Ancak diğer çalışmalardaki farklı yaklaşımlar ve tartışmalı sonuçlar nedeniyle, tek başına raloksifenin genitoüriner sistem üzerindeki etkilerini belirlemek amacıyla daha geniş katılımlı, daha uzun izlem süreli ve plasebo kontrollü çalışmalara ihtiyaç vardır.en_US
dc.description.abstractObjective: To evaluate the effect of raloxifene on urogenital symptoms and sexual function in women who had never taken any topical estrogen treatment. Material and Methods: Raloxifene was given to 44 patients who had at least three-year amenorrhoea and who did not want to use hormone replacement therapy, and 46 patients were selected as a control group. Sexual function score was determined according to the sexual desire, coitus frequency, sexual satisfaction, vaginal dryness and dysparenuia. The vaginal atrophy score was determined according to the state of vaginal rugae, mucosal color, the presence of petechia and elasticity. Urge and stress incontinence, the frequency and severity of incontinence were primarily evaluated according to history, pelvic examination and cough stress test. Results: The sexual function score was diminished after six months therapy compared with the basal values (P = 0.002), but this diminution was not different between the groups after six-months therapy (P > 0.05). Although there was an increase in vaginal atrophy score, this increase was not statistically significant and there was no difference between the groups (P > 0.05). There were also no differences according to the types of incontinence, the frequency and severity of incontinence compared with the basale values and within the groups (P>0.05). Conclusion: In this study, raloxifene has negative effects on sexual dysfunction, but no effect on vaginal mucosa and the structure of pelvic floor. However, because of different interventions and conflicting results large, long-lasting and placebo-controlled studies are required to determine the effects of raloxifene on genitourinary system.en_US
