Özet
Amaç: Çalısmamızda Orak Hücre Anemi (OHA) tanılı hastalarda orbital doppler ultrasonografi ve optik sinir Shear Wave Elastografi (SWE) bulgularının degerlendirilmesi amaçlandı. Gereç ve Yöntem: Çalışmamız prospektif tasarımda yapılmıştır. Çalışmaya 18 yaş ve üzeri 30 OHA tanılı hastanın toplam 60 gözü dahil edildi. Hasta grubuyla yaş ve cinsiyet açısından benzer olan 30 sağlıklı gönüllü bireyin toplam 60 gözü ise kontrol grubu olarak belirlendi. Tüm bireylere Hatay Mustafa Kemal Üniversitesi Tıp Fakültesi Radyoloji Anabilim Dalı'nda orbital doppler USG ve SWE yapıldı. Oftalmik arter (OA), posterior siliyer arter (PSA) ve santral retinal arter (SRA) için pik sistolik hız (PSV) ve diyastol sonu hızları (EDV) ölçüldü ve Resistivite indeksi (RI) hesaplandı. SWE optik sinir çapı, optik sinir velocity ve optik sinir stiffness değerleri ölçüldü. Orbital doppler USG ve SWE bulguları hasta ve kontrol grupları arasında karşılaştırıldı. Bulgular: OHA grubunda bilateral OA, PSA ve SRA'nın hem PSV hem de EDV degerlerinin kontrol grubundan yüksek oldugu saptandı (p<0,05). OHA hastalarında bilateral OA, PSA ve SRA'nın RI degerlerinin kontrol grubundan düşük oldugu saptandı (p<0,05). OHA grubunda bilateral optik sinir velocity ve stiffness degerlerinin kontrol grubundan yüksek oldugu (sırasıyla; p<0,001, p<0,001), optik sinir çapının ise OHA grubunda kontrol grubundan anlamlı olarak düşük olduğu saptandı (p=0,038). Hasta grubunda OA'in PSV ile optik sinir velocity ve stiffness değerleri arasında pozitif yönde orta düzeyde korelasyon saptandı (sırasıyla; r=0,407, p=0,025; r=0,394, p=0,031). Hasta grubunda OA EDV ile optik sinir velocity ve optik sinir stiffness değerleri arasında pozitif yönde orta düzeyde korelasyon saptandı (sırasıyla; r=0,456, p=0,011; r=0,495, p=0,005). Sonuç: OHA hastalığında oküler arterlerde meydana gelen değişikliklerin orbital doppler USG ve SWE ile noninvaziv ve güvenli olarak kolayca ölçülebileceği ve orbital doppler USG ve SWE'nin OHA hastalığında göz tutulumunun erken tanınması, seyir ve prognozunun degerlendirilmesinde tanıya katkı saglayabilecek yöntemler oldugu düşünüldü.

Materials and Methods: Our study is a prospective design study. A total of 60 eyes of 30 patients aged 18 years and older with a diagnosis of SCA were included in the study. A total of 60 eyes of 30 healthy volunteers who were similar to the patient group in terms of age and gender were determined as the control group. Orbital Doppler USG and SWE were performed on all individuals in Hatay Mustafa Kemal University, Faculty of Medicine, Department of Radiology. Peak systolic velocity (PSV) and end-diastolic velocities (EDV) were measured for the ophthalmic artery (OA), posterior ciliary artery (PCA), and central retinal artery (CRA). Resistivity index (RI) was calculated. SWE optic nerve diameter, optic nerve velocity and optic nerve stiffness values were examined. Orbital Doppler USG and SWE findings were compared between the patient and control groups. Results: Both PSV and EDV values of bilateral OA, PCA and CRA were found to be higher in the SCA group than in the control group (p<0.05). RI values of bilateral OA, PCA and CRA were found to be lower in SCA patients than in the control group (p<0.05). In the SCA group, bilateral optic nerve velocity and nerve stiffness values were found to be higher than the control group (p<0.001, p<0.001, respectively), and the optic nerve diameter was found to be significantly lower in the SCA group than the control group (p=0.038). There was a moderate positive correlation between OA PCV and optic nerve velocity and optic nerve stiffness values in the patient group (respectively; r=0.407, p=0.025; r=0.394, p=0.031). There was a moderate positive correlation between OA EDV and optic nerve velocity and optic nerve stiffness values in the patient group (respectively; r=0.456, p=0.011; r=0.495, p=0.005). Conclusion: The changes in the ocular arteries in SCA can be easily measured noninvasively and safely with orbital Doppler USG and SWE and orbital Doppler USG and SWE were thought to be methods that could contribute to the diagnosis in the early recognition of eye involvement, course and prognosis of SCA.
