Bir Sansarda Desflurane Anestezisi eşliğinde Antebrachium kırığının tedavisi

dc.description.abstractBu olgu sunumunun materyalini Mustafa Kemal Üniversitesi Veteriner Fakültesi kliniklerine getirilen 2 yaşındaki, 520 gr erkek bir sansar oluşturdu. Klinik muayenede; bilinç kaybı, sallantılı yürüme, topallık, iştahsızlık, halsizlik, dehidrasyon, yüksek ateş ile karakterize şok tablosu belirlendi. Radyolojik muayenede; sağ ön kolda diafizer transversal antebrachium (radius ve ulna) kırığı ve oblik metacarpus IV kırığı saptandı. Hastanın genel durumu sıvı tedavisi, antibiyotik ve kortikosteroid uygulaması ile düzeltildi. Kalp ve solunum fonksiyonları operasyon öncesi, sırası ve sonrasında kontrol edildi. Anestezi indüksiyonu intramusküler 0.1 mg.kg-1 medetomidine HCL ve 25 mg.kg -1 ketamine HCL ile gerçekleştirildi. Anestezinin devamlılığı %30 oksijen ve %9-12 end-tidal desflurane konsantrasyonda sürdürüldü. Anestezi öncesindeki dakikada kalp atımı (Hr:160), solunum sayısı (Rr:50), arteriyel oksijen hemoglobin saturasyonu (SpO 2:94) ve vücut ısısı (T:380C) değerleri medetomidine+ketamin (Hr:72, Rr:30, SpO2:90, T:370C) ve desflurane uygulaması (Hr:128, Rr:18, SpO2:88, T:360C) sırasında azaldı. Açık redüksiyonla kırık hattına ulaşıldı ve 0.6 mm çaplı serklaj teli ile kırık fragmentler tesbit edildi. Metacarpus IV kırığı için bandaj uygulandı. Operasyondan sonra hayati fonksiyonlar normal düzeylerine (Hr:137, Rr:40, SpO2:98, T:360C) döndü. Postoperatif 30 gün sonra alçılı bandaj çıkarıldı. Sansar düzgün yürümeye başladı. Doğal yaşamına dönmesi için ormanlık bir alanda serbest bırakıldı. Sonuç olarak; sansarda desflurane anestezisi sonrası kendine gelme süresi evcil karnivorlardan daha uzun olduğu ve % 9-12 end-tidal desflurane konsantrasyonu ortopedik cerrahide güvenli bir şekilde kullanılabilir.en_US
dc.description.abstractThe material of this case report was a male Martes with two-year-old and 520-gram-weigh brought to the Department of Surgery, Faculty of Veterinary Medicine, Mustafa Kemal University. In clinical examination, a traumatic shock statue characterized with unconsciousness, wobble gait, lameness, anorexia, protration, dehydration and high fever was determined. Radiological examination showed the diaphyseal transverse fracture of antebrachium (radius and ulna) and an oblique fracture of metacarpus IV in the right front arm. The patient's general condition improved with fluid therapy, antibiotics and corticosteroid applications. Heart and lung functions were controlled before, during and after surgery. Anesthesia induction was performed with intramuscular medetomidine HCL (0.1 mg.kg-1) and ketamine HCl (25 mg.kg-1). Anesthesia was maintained with 30% oxygen and 9% and 12% end-tidal desflurane concentration. The values of heart rate (Hr:160), respiratory rate (Rr:50), arterial hemoglobin oxygen saturation (SpO2:94) and body temperature (T:38C) prior to anesthesia decreased during medetomidine+ketamine (Hr:72, Rr:30, SpO 2:90, T:37C) and desflurane application (Hr:128, Rr:18, SpO2:88, T:36C). The fracture site was exposed with open reduction and the fragments were fixed by cerclage wire of 0.6 mm in diameter. The fracture of metacarpus IV was immobilised with bandage. The vital functions returned to normal levels after operation (Hr:137, Rr:40, SpO2: 98, T:36 C). The plaster bandages were removed postoperatively after 30 days. The Marten began to walk properly, which was then released back to its natural habitant in a forested area. In conclusion, the recovery time after desflurane anesthesia is longer than domestic carnivores in the marten, and desflurane (9-12% end-tidal) can be used safely in orthopedic surgery.en_US
