Özet
Bu araştırma; Hemodiyaliz hastalarına tele-hemşirelik yoluyla verilen eğitim ve danışmanlığın hastalık yönetimine, yaşam kalitesine ve öz bakım davranışına etkisini değerlendirmek amacıyla randomize kontrollü deneysel araştırma tasarımı kullanılarak yapıldı. Araştırma; Hatay ilinin Kırıkhan ilçesinde Kırıkhan Devlet Hastanesi HD Ünitesi'nde Eylül 2022- Haziran-2023 tarihleri arasında yapıldı. Verilerin toplanmasında; hasta bilgi formu, metabolik kontrol izlem formu, Hemodiyaliz Hastalarının Diyet Bilgi Ölçeği(HHDBÖ), Hemodiyaliz Hastalarının Diyet Davranış Ölçeği (HHDDÖ), Diyaliz Semptom İndeksi(DSİ), Hemodiyaliz Hastalarında Sıvı Kontrol Ölçeği(HHSKÖ), Hemodiyaliz Hastalarında Arteriyovenöz Fistüle İlişkin Öz Bakım Davranışlarını Değerlendirme Ölçeği, Böbrek Hastalığı Yaşam Kalitesi-36 Ölçeği, tele-hemşirelik memnuniyet değerlendirme formu kullanılmıştır. Araştırma sonunda yapılan güç analizinde veri karşılaştırmalarının sonuçları %95, %99 güven aralığında; p<0,05, p<0,01 anlamlılık düzeyinde değerlendirilmiştir. Araştırmada katılımcıların HHDBÖ, HHDDÖ, HHSKÖ ve DSİ ön test-son test ölçümleri arasında fark olduğu saptanmıştır. Deney grubuna uygulanan tele-hemşirelik ile izlem sonucunda HHDBÖ, HHDDÖ ve HSKÖ puan ortalamalarının arttığı, kontrol grubunda ise düşüş olduğu, DSİ puan ortalamalarının deney grubunda düştüğü, kontrol grubunda arttığı görülmüştür. HD Hastalarında Arteriyovenöz Fistüle (AVF) İlişkin Öz Bakım Davranışlarını Değerlendirme Ölçeği ön test-son test ölçümleri arasında fark olduğu deney grubunun uygulanan tele-hemşirelik ile izlemi sonucunda AVF ilişkin öz bakım davranışlarının arttığı, kontrol grubunda ise puan ortalamalarının düştüğü saptanmıştır. Böbrek Hastalığı Yaşam Kalitesi-36 Ölçeği; Fiziksel İşlevler alt boyutu, Mental İşlevler, Böbrek Hastalığı Yükü, Semptomlar ve Problemler, Günlük Yaşam Etkileri ölçümleri arasında fark olduğu, deney grubunun uygulanan tele-hemşirelik ile izlemi sonucunda tüm alt boyutlarda puan artışı olduğu, kontrol grubunda ise puan ortalamalarının düştüğü gözlemlenmiştir. Sonuç olarak HD hastalarına tele-hemşirelik yoluyla verilen eğitim ve danışmanlığın hastalık yönetimine, yaşam kalitesine ve öz bakım davranışına olumlu yönde etki ettiği ve tele-hemşirelik destekli bireysel eğitiminin HD tedavisi alan hastalarda kullanılabilir bir uygulama olduğu sonucuna ulaşılmıştır.
This research was conducted using a randomized controlled experimental research design with the aim of evaluating the impact of training and counseling provided to hemodialysis patients via tele-nursing on disease management, quality of life, and self-care behavior. The research was conducted between September 2022 and June 2023 in the Hemodialysis Unit of Kırıkhan State Hospital in Kırıkhan district of Hatay province. In the process of data collection, the following instruments were employed: a patient information form; a metabolic control monitoring form; the Scale for Dietary Knowledge in Hemodialysis Patients (SDKHP); the Attitude Scale for the Dietary Therapy of Hemodialysis Patients (ASDTHP), the Dialysis Symptom Index (DSI), the Fluid Control in Hemodialysis Patients Scale (FCHPS), the Scale for the Assessment of Self-Care Behaviors with Arteriovenous Fistula in Hemodialysis, the Kidney Disease Quality of Life (KDQOL-36) survey, and a tele-nursing satisfaction evaluation form. At the end of the research, the results of data comparisons were evaluated with a confidence interval of 95% and 99%, considering significance levels of p<0.05 and p<0.01. In the study, differences were observed in the pre-test and post-test measurements of SDKHP, ASDTHP, FCHPS, and DSI among the participants. Following the application of tele-nursing to the study group, an increase in the mean scores of SDKHP, ASDTHP, and FCHPS was observed, while in the control group, a decrease was noted. Additionally, the mean scores of the Dialysis Symptom Index decreased in the study group and increased in the control group. The Scale for the Assessment of Self-Care Behaviors with Arteriovenous Fistula (AVF) in Hemodialysis Patients revealed a difference between the pre-test and post-test measurements. As a result of the tele-nursing intervention and follow-up on the study group, an increase was observed in self-care behaviors related to AVF, whereas in the control group, average scores decreased.The Kidney Disease Quality of Life-36 Scale demonstrated differences among its subscales, including Physical Functioning, Mental Functioning, Kidney Disease Burden, Symptoms and Problems, and Daily Life Impact measurements. Following the application of tele-nursing and subsequent follow-up on the study group, an increase in scores was observed across all subscales. In contrast, the control group exhibited decreased average scores.In conclusion, the training and counseling provided to hemodialysis patients through tele-nursing demonstrated a positive impact on disease management, quality of life, and self-care behavior. The findings suggest that individually customized tele-nursing training presents a feasible intervention for individuals undergoing hemodialysis treatment.
