dc.description.abstractAmaç: : Elektif TUR-P (Transüretral Prostat rezeksiyonu) cerrahisi geçirecek geriatrik yaş grubundaki (>65 Yaş) hastalarda DAA uygulamalarının POKD üzerine etkisinin araştırılması birincil olarak amaçlanmıştır. İkincil amacımız ise POKD ile ilişkisi olan faktörlerin ortaya konmasıdır. Yöntem: Etik Kurul iznini takiben, prospektif deneysel olarak tasarlanan çalışmaya genel anestezi altında elektif TUR-P operasyonuna girecek 65 yaş ve üzeri 98 hasta dahil oldu. Operasyondan 1.gün öncesinde ve postoperatif dönemde, 6. saat,.1. gün,.3. gün ve.7. gün MMT ile değerlendirilip kayıt altına alındı. VAS skorları 3, 6, 12, 24, 48 ve 72. saatte ölçüldü ve kayıt altına alındı. Perioperatif dönemde özefagus ısı probu kullanılarak hastaların vücut ısı değerleri ölçüldü ve kayıt altına alındı. Kaydedilen veriler DAA uygulanan (1 L/dk, N=49) ve YAA 4 L/dk uygulanan (N=49) hasta grupları arasında karşılaştırıldı. P< 0.05 İstatiksel olarak anlamlı kabul edildi. Bulgular: Çalışmaya dahil olan hastaların.demografik verileri ( yaş, cinsiyet ve American Society of Anesthesiologists (ASA) skorları, VKİ (Vücut kitle indexi), operasyon süresi) arasında.fark saptanmadı. 6.saat, 1.gün, 3.gün ve.7.gün MMT skorları DAA ve YAA grupları arasında karşılaştırıldığında anlamlı bir fark bulunmadı (p=0,668, 0,785, 0,745, 0,705 ). Uygulanan akım yöntemlerine göre olguların 3, 6, 12, 24, 48, 72.saat VAS skorları.istatistiksel.olarak.anlamlı farklılık.göstermemektedir (p=0,316, 0,925, 0,651, 0,548, 0,624, 0,466). Ortalama Arteryel Basınç (OAB), Ortalama Nabız ve Göz açma sürelerinin her iki grupta karşılaştırılmasında anlamlı.bir.farklılık.saptanmadı (p=0,369, 0,520, 0,340). Her iki grup perioperatif ısı değerleri karşılaştırıldığında 30, 60, 90.dk' larda DAA grubunda YAA 'a göre ısı değerlerinin daha iyi korunduğu bulunmuştur (p<0.001). Sonuç: Sonuç olarak; düşük akım anestezinin gerekli ekipman varlığında güvenle uygulanabileceğini fakat yüksek akıma kıyasla kognitif fonksiyonlarda belirgin bir etkisinin olmadığını düşünüyoruz. Ancak uzun dönem takiplerde etkisinin araştırılmasının litaretüre katkıda bulunacağını kanaatindeyiz.en_US
dc.description.abstractBackground and aim: It is primarily aimed.to.investigate.the.effect of LFA (Low.flow anaesthesia) applications on POCD in.patients in the geriatric age group (>65 years old) who will undergo elective TUR-P (Trans-urethral Resection Prostate) surgery. Our secondary aim is to reveal the factors associated with POCD. Methods: Following the approval of the Ethics Committee, 98 patients aged 65 years and older who would undergo elective TUR-P operation under general anesthesia were included in the study, which was designed as a prospective experimental study. 1st day before the operation and in the postoperative period, 6th hour,.1. day,.3. day and.7. The day was evaluated and recorded with MMT. VAS scores were measured and recorded at 3, 6, 12, 24, 48 and 72 hours. In the perioperative period, the body temperature values of the patients were measured and recorded by using the esophageal temperature probe. Recorded data were compared between the groups of patients who received DAA (1 L/min, N=49) and 4 L/min LAA (N=49). P< 0.05 was considered statistically significant. Results:There was no difference between the demographic data (age, gender and American Society of Anesthesiologists (ASA) scores, BMI (Body mass index), operation time) of the patients included in the study. When the 6th hour, 1st day, 3rd day and 7th day MMT scores were compared between LFA and HFA groups, no significant difference was found (p=0.668, 0.785, 0.745, 0.705). According to the applied flow methods, the 3, 6, 12, 24, 48, 72. hour VAS scores of the cases did not show any statistically significant difference (p=0.316, 0.925, 0.651, 0.548, 0.624, 0.466). No significant difference was found in the comparison of Mean Arterial Pressure (MAP), Mean Pulse, and Eye opening times in both groups (p=0.369, 0.520, 0.340). When the perioperative temperature values of both groups were compared, it was found that the temperature values of the LFA group were better preserved at the 30th, 60th, and 90th minutes compared to the HFA (p<0.001). Conclusion: As a result; Our opinion that low flow anesthesia can be applied safely in the presence of necessary equipment, but it does not have a significant effect on cognitive functions compared to high flow. However, we believe that investigating its effect in long-term follow-ups will contribute to the literature.en_US
