dc.description.abstractGiriş ve Amaç: M. tuberculosis ile enfeksiyonun belirlenmesinde yaygın olarak yararlanılan tüberkülin cilt testinin BCG aşılı kişilerde ve tüberküloz dışı mikobakteri infeksiyonu olasılığında yanlış pozitiflikler nedeniyle yorumlanması güç olmaktadır. Son yıllarda bu gibi zorlukları giderebilecek, IFN-? salınımının ölçümüne dayanan in vitro testler geliştirilmiştir. Bu çalışmada LTBE açısından araştırılan farklı risk gruplarındaIFN-? salınım testi sonuçlarının TCT sonuçları ile karşılaştırılması planlanmıştır.Gereç ve Yöntem: Yaşları21 ile 90arasındaki89 olgu 3 gruba ayrılmıştır. Olguların kollarının volar yüzeyine intradermal olarak TCT yapılmış ve test 48?72 saat sonra değerlendirilmiştir. QuantiFERON-TB Gold in Tube testi için olgulardan venöz kan alınarak periferik kan T hürelerinde M. tuberculosis'e özgül ESAT-6, CFP-10 ve TB7.7 antijenlerine karşıönceden enfeksiyon yoluyla duyarlanma olup olmadığıaraştırılmıştır.Bulgular: Çalışmaya alınan 98 hastanın 9'undaQFT-GIT testi ``belirsiz'' saptandığından çalışmadan çıkarıldı. 89 hastanın 34'ünde TCT pozitif iken, 30 hastada QFT-GIT testi pozitif idi. BCG skar sayısı ile TCT çapları arasında anlamlı ilişki bulunamadı (Pearson korelasyon testi 0,195 p=0,067).Tüm olgular içinde TCT ile QFT-GIT testi arasındaki uyum orta olarak değerlendirildi (? değeri =0,708). Her üç grupta da TCT ile QFT-GIT düzeyi arasında pozitif korelasyon saptandı. Latent tüberküloz enfeksiyonu tanısında TCT için sınır değer ?15 mm alınarak QFT-GIT testinin sensitivite ve spesifitesi hesaplandı. Tüm olgular içinde sensitivite % 76,4; spesifite % 92,7 olarak bulundu.Sonuç: Ülkemizde olduğu gibi, BCG aşısının rutin olarak uygulandığıve TCT yanlışpozitiflik oranının yüksek olduğu topluluklarda IFN-?salınımına dayanan testlerleriskli gruplarda LTBE tanısıdaha spesifik olarak konulabilecek; böylece İzoniazid profilaksisininyerinde kullanımısağlanabilecektir.en_US
dc.description.abstractIntroduction:It is hard to interpret the result of tuberculin skin test which is widely used in determining the M. tuberculosis and infection because of false positivity in BCG vaccinated people and in the possibility of none- tuberculosis mycobacterial infections. In recent years, some in-vitro tests have been developed based on the measurement of IFN-? release in order to overcome this difficulty. In this study, we examined the comparison of theIFN-?release assays with TST results in different risk groups that are researched in terms of LTBI.Materials and Methods:89 subjects aged between 21 ? 90 were seperated into three groups. TST was intradermatologically applied to volar surface of arms, and the result was evaluated 48-72 hours later. For QuantiFERON-TB Gold in Tube test, subjects were drawn venuos blood and we intestigated for availability of sensitivity in peripheral blood cells due to previous infection towards ESAT-6, CFP-10 and TB7.7 antigens which are specific to M. tuberculosis.Results:9 patients out of 98 were excluded from the study as the QFT-GIT test was determined ?indeterminate?. QFT-GIT test was positive in 30 patients whereas TST was positive in 34 out of 89. There was no meaningful relation between BCG scar number and TST scale (Pearson correlation test 0,195 p=0,067). In all patients, the consistency was determined medium between TST and QFT-GIT test. (? value =0,708). Positive correlation was found in all three groups between TST and QFT-GIT level. The limit value in the diagnosis of latent TB infection was set ?15 mm and we estimated the sensivitiy and specificity of QFT-GIT test. The sensitivity was % 76,4; and specificity was % 92,7 in all cases.Conclusion:As in our country, LTBI diagnosis is likely to be determined more specifically in risky groups due to tests based on IFN-?release in communities where BCG vaccination is routinely applied and the rate of TST false positivity is high, and isoniazid prophylaxis will, therefore, be used appropriately.Key words:Latent tuberculosis, tuberculin,QuantiFERON-TB Gold In Tubeen_US
