Öğe Androgen status of the nonautoimmune dry eye subtypes(Karger, 2006) Tamera, Cengaver; Oksuz, Huseyin; Sogut, SadikPurpose: To evaluate androgen levels of patients diagnosed with nonautoimmune dry eye, either with meibomian gland dysfunction (MGD) or without MGD (non-MGD), and normal control subjects. This is a prospective, comparative, case-control study. Methods: Sixty-four (32 men and 32 women) subjects were enrolled for each of the three diagnostic groups. All dry eye patients were symptom positive. Non-fasting testosterone (T), sex hormone-binding globulin, serum albumin, dehydroepiandrosterone (DHEA), and DHEA sulphate levels of all study participants were determined using either automated immunoenzymatic assay, or standard radioimmunoassay. Analysis of variance was used to compare androgen levels among the three diagnostic groups in a gender-based design, followed by post-hoc multiple comparisons with the Tukey honestly significant difference test. Results: Mean T levels in men and women of the three diagnostic groups were not significantly different (p = 0.808, p = 0.156, respectively; ANOVA). Statistical analyses of the three diagnostic groups revealed a significant difference for men and women in bioavailable T levels (p = 0.002, p = 0.014, respectively; ANOVA), DHEA levels (p = 0.009, p = 0.004, respectively; ANOVA), and DHEA sulphate levels (p = 0.001, p = 0.001, respectively; ANOVA), whereas there was no statistically significant difference between non-MGD dry eye patients and controls for any of the measured androgen levels according to the post-hoc tests. Conclusion: This study demonstrates that the androgen pool of nonautoimmune dry eye patients with MGD is significantly depleted compared with that of non-MGD and control cases. Copyright (c) 2006 S. To report a case of dacryocystitis that progressed to orbital abscess resulting in the rare complication of central retinal artery occlusion. Methods. A patient without any other known health problems presented with dacryocystitis complicated by orbital cellulitis, orbital abscess, and finally central retinal artery occlusion despite medical treatment. Results. After 48 hours of medical treatment, there was no recovery, and there was loss of light perception; because of this, surgery indication was determined. Fundus fluorescein angiogram demonstrated findings of central retinal artery occlusion. There was no recovery of light perception after surgical intervention. Conclusions. We report a case of orbital abscess following dacryocystitis in a healthy middle-aged woman resulting in the rare complication of central retinal artery occlusion. To our knowledge, this is the first report of central retinal artery occlusion secondary to orbital abscess following dacryocystitis.Öğe Circadian intraocular pressure control with dorzolamide versus timolol maleate add-on treatments in primary open-angle glaucoma patients using latanoprost(Karger, 2007) Tamer, Cengaver; Oksuz, HuseyinPurpose: To compare the 24- hour efficacy of dorzolamide and timolol maleate administered twice daily to primary open-angle glaucoma patients whose intraocular pressure ( IOP) could not be adequately controlled with latanoprost monotherapy. Methods: In this double- blind prospective crossover clinical comparison trial, 36 primary open- angle glaucoma patients with uncontrolled IOP despite treatment with latanoprost applied once daily were administered timolol and dorzolamide twice daily. The treatment sequence was randomized. All patients underwent measurements for four 24- hour tonometric curves: at baseline and after each 4- week period of treatment. The IOP measurements were taken at 06: 00, 09: 00, 12: 00, 15: 00, 18: 00, 21: 00, 24: 00 and 03: 00 h. The between- group differences were tested for significance by means of parametric analysis of variance at each time point and circadian curve. The peak values within circadian curve were defined. The mean of the exact amount and percentage of additional IOP reductions from baseline were evaluated and success rates ( a minimum of 10% reduction) were determined for both drug regimens. Results: The mean peak/ circadian curve IOPs were 23.4 +/- 8 2.2/ 21.8 +/- 8 2.2 mm Hg at dorzolamide baseline, 23.3 +/- 8 2.2/ 21.7 +/- 8 2.1 mm Hg at timolol baseline, and reduced to 20.2 +/- 8 1.7/ 18.7 +/- 8 1.7 mm Hg and 20.7 +/- 8 2.4/ 19.4 +/- 8 1.6 mm Hg, respectively. When added to latanoprost, both dorzolamide and timolol lowered IOP at circadian curve significantly ( p < 0.05). Dorzolamide reduced baseline IOP values at each time point. Timolol also significantly reduced baseline IOP values at all time points except at 03:00. The mean of the exact amount and percentage of reduction in IOP at circadian curve and 5 out of 8 time points were significantly greater with dorzolamide add-on treatment ( p < 0.05). The successful reduction rates were 86% for the dorzolamide group and 61% for the timolol group ( p = 0.016; chi(2) 2 test). Conclusion: Both of the combinations are effective in lowering IOP, the exact amount and percentage of reduction is greater with the latanoprost + dorzolamide regimen, especially at night- time. Karger AG, Basel.Öğe Comparative trial of different anti-bacterial combinations with propolis and ciprofloxacin on Pseudomonas keratitis in rabbits(Elsevier Gmbh, 2007) Onlen, Yusuf; Tamer, Cengaver; Oksuz, Huseyin; Duran, Nizami; Altug, Mehmet Enes; Yakan, SelvinazThe aim of the current study was to evaluate the effects of five different treatment combinations to find out whether propolis could be an alternative or an adjunctive treatment, in experimental Pseudomonas aeruginosa keratitis. Intrastromal P aeruginosa strains were given to both eyes of 20 young New Zealand white rabbits. The rabbits were randomly divided equally into five treatment groups; ciprofloxacin and dexamethasone drops (C+D), ciprofloxacin drop (C), ciprofloxacin and propolis drops (C+P), propolis drop (P), 3% ethanol drop (control), respectively. Directly before the first treatment and 108h after inoculation, the eyes were examined by slit lamp to assess the corneal opacity and rabbits were sacrificed for bacterial count. The mean corneal opacity scores and the mean bacterial counts log cfu/ml were significantly different in the treatment groups (P = 0.001; ANOVA). According to post hoc tests for both the mean bacterial counts and corneal opacity scores, C+D, C, C+P groups were found to be statistically the same (P > 0.05), and although the P group had significantly better scores than the control group it did not reach the scores of the rest of the treatment groups (P < 0.01). We conclude that propolis may be a useful adjunctive agent but should not be regarded as a replacement for traditional antibiotic therapy for P aeruginosa keratitis in rabbits. We hypothesized that patients who undergo endoscopic dacryocystorhinostomy may also experience mucociliary dysfunction. We evaluated the effects of surgery on mucociliary clearance time in patients who underwent endoscopic dacryocstorhinostomy and external dacryocstorhinostomy. STUDY DESIGN: A prospective controlled study. SUBJECTS AND METHODS: Forty-four patients were studied: 22 had undergone one-sided endoscopic Surgery and 22 had undergone one-sided external surgery for nasolacrimal duct obstruction. Nonoperated sides of the both groups were remained as controls. The saccharin test was used to evaluate the mucociliary clearance time of the both sides. RESULTS: Dacryocystorhinostomy, results in prolonged mucociliary clearance times either endoscopic or external approach is performed (P < 0.001, paired t test). There was no significant difference between the mucociliary clearance times after endoscopic and external dacryocystorhinostomy), (P = 0.22, Student's t test). CONCLUSION: Dacryocystorhinostomy impairs mucociliary function independent of the type of surgery.Öğe Effects of the Bovine Amniotic Membrane on Corneal Healing with Acute Alkali Burns in Rabbits(Medwell Online, 2009) Gonenci, Ramazan; Altug, Muhammed Enes; Koc, Ahmet; Oksuz, Huseyin; Yuksel, HayatiThe Bovine Amniotic Membrane (BAM) was transplanted on the right comea of 8 adult rabbits after an alkali burn was created with 2.5 N NaOH. Density and area of the corneal opacity, area of the corneal epithelial defect, vascularization and complications were clinically evaluated. Left corneas of the same animals were used as active control group. At the end of the experiment, all the eyes were extirpated and histologically examined on light microscopy. BAM has decreased density and area of the corneal opacity and increased reepithelialisation, but promoted episcleral vasodilatation more prominently in clinical examination. Histologically; the distrubition of Polymorphonuclear (PMN) leucocytes, corneal thicknesses, edema, keratinization, neovascularization and inflammation were significantly reduced by BAM compared to active control group. Material and Method: Topical phenylephrine HCI 2.5% was administered 3 times with 5-minute intervals before cataract surgery to 44 eyes of 44 patients. The systemic blood pressure was measured preoperatively and at 15 and 30 minutes. Results: The mean systolic blood pressure before the topical administration of phenylephrine HCI 2.5% was 119.7mmHg, while 15 and 30 minutes later, it was 125.3 mmHg and 121.2 mmHg, respectively. The mean diastolic blood pressure before the topical administration of phenylephrine HCI 2.5% was 75.9 mmHg; 15 and 30 minutes later, it was 76.9 mmHg and 75.3 mmHg, respectively. The increase in systolic blood pressure at 15 and 30 minutes was statistically significant (p=0.003 and p=0.042). The change in diastolic blood pressure was not statistically significant. Discussion: Phenylephrine HCI 2.5%, which is used preoperatively as a fast and effective mydriatic for cataract surgery, can increase the systolic blood pressure. This effect is most significant at 15 minutes and lasts at least 30 minutes. This agent should be used with care in hypertensive patients.Öğe Endoscopic dacryocystorhinostomy with silicone, polypropylene, and T-tube stents; randomized controlled trial of efficacy and safety(Ocean Side Publications Inc, 2015) Okuyucu, Semsettin; Gorur, Hatice; Oksuz, Huseyin; Akoglu, ErtapObjective: To compare the efficacy and safety of endoscopic dacryocystorhinostomy (En-DCR) with different stent materials for lacrimal sac intubation inprimary nasolacrimal ductal obstructions. Study Design: Randomized controlled study with three parallel groups. Level of evidence is 1b. Methods: A total of 91 patients (five bilateral) with primary nasolacrimal duct obstruction (NLDO) at a tertiary referral center scheduled for En-DCR were to allocated into three stent groups with a sealed envelope and were randomized into three treatments: silicone, Prolene (polypropylene), and otologic T-tube. Ophthalmology and otolaryngology clinics evaluated the patients preoperatively and postoperatively with endoscopes, lacrimal system syringing, and dacryocystography. The success of the stents was evaluated 12 months after surgery with symptom relief and ostial patency. Complications were also noted. Results: The overall success rate of the En-DCR in the stent groups was 78.1% (75/96); specifically, 87.5% (28/32) with silicone, 84.4% (27/32) with Prolene, and 62.5% (20/32) with T-tube. The efficacy of the procedures with the T-tube was significantly lower than that of the Prolene and silicone (p =.031, x(2) test). There were no significant differences between the silicone and Prolene (p =.718, x(2) test). Prolene was found to be related with orbital complications. Spontaneous loss is a particular complication of otologic T-tube and highly portends to failure. Prolene (R) is cheap, effective, and readily available in almost all operating theaters. It might be used successfully in endoscopic dacryocystorhinostomy and is promising as an alternative to silicone stent intubations, especially in settings with limited resources. Objective. To evaluate the clinical efficacy and results of stenting with polypropylene (Prolene (R),Ethicon, Inc.) suture material instead of silicone tube in endoscopic dacryocystorhinostomy. Patients and methods. Forty-two endoscopic dacryocystorhinostomy operations were performed in 36 patients (I I men and 25 women; mean age 34.7 +/- 9.5 years, range 16-60 years) between 2007 and 2008. After creating an aperture in the medial wall of the lacrimal sac, 2/0 Prolene was inserted through the canaliculi into the sac. The Prolene was left in the lacrimal sac for 3 months. The patients were followed up for between 6 and 18 months (mean 8.1 +/- 3.6 months). The improvement in the complaint of epiphora was grouped as very good, good, or no change. Results. The improvement in 34 eyes (81%) was reported as very good, whereas improvement was noted as good in 5 eyes (11.9%), and there was no change in 3 eyes (7.1%). Methods: We present four cases of traumatic iridodialysis that were repaired by 8-0 polypropylene suture. Results: Better iris reposition and stability were achieved with 8-0 polypropylene suture despite wide degree traumatic iridodialysis in all patients. Conclusion: In this study, we used 8-0 polypropylene suture, which is thicker, cheaper, and more durable than 10-0 sutures. We suggest that usage of 8-0 polypropylene sutures may provide better iris repositioning.Öğe Serum dehydroepiandrosterone sulphate level in age-related macular degeneration(Elsevier Science Inc, 2007) Tamer, Cengaver; Oksuz, Huseyin; Sogut, SadikPURPOSE: To evaluate plasma dehydroepiandrosterone sulphate (DHEAS) levels in patients diagnosed with age-related macular degeneration (AMD) and controls. DESIGN: Case-controlled, prospective, comparative noninterventional study. METHODS: This study involved 32 men and 35 women with exudative AMD, 37 men and 38 women with nonexudative AMD, and 32 men and 32 women of an age,matched control group. The Wisconsin Age,Related Maculopathy Grading System was used to asses the severity of AMD lesions. DHEAS levels were measured and compared according to a gender based subdivision Analysis of variance was used to assess the association between DHEAS and AMD. Linear regression model was used to examine the relation among DHEAS level and AMD severity scale. RESULTS: Mean +/- SD of DHEAS levels in exudative AMD, nonexudative AMD, and controls in men was 2.67 +/- 0.68 mu mol/l, 2.89 +/- 0.95 mu mol/l, and 4.43 +/- 1.44 mu mol/l, respectively (P =.001), and in women was 1.64 +/- 0.72 mu mol/l, 1.85 +/- 0.73 mu mol/l, and 2.78 +/- 0.91 mu mol/l, respectively (P = .001). Post hoc Tukey analyses revealed a significant reduction in serum DHEAS level in both AMD groups, compared with controls for men and women (P =.001), while no difference was found between AMD groups in both men and women (P = .668 and 0.49, respectively). Regres, sion analyses revealed an inverse correlation among serum DHEAS level and AMD severity scale both in men and women (P = .006 and .007, respectively). CONCLUSIONS: This study suggests an inverse correlation between serum DHEAS level and AMD severity scale with a considerably reduced DHEAS level in AMD.Öğe Terson Syndrome (A case Report)(Turkish Ophthalmological Soc, 2012) Ilhan, Ozgur; Ilhan, Nilufer; Coskun, Mesut; Ayintap, Emre; Keskin, Ugurcan; Semiz, Hilal; Oksuz, HuseyinTerson syndrome is a vitreous hemorrhage that occurs after intracranial hemorrhages (subarachnoid or subdural) and generally visual deficit or low vision is observed. In this paper, the case of a patient who had undergone pars plana vitrectomy due to Terson syndrome is presented. Visual acuity of the patient was 0.1 prior to the operation, but 1 month postoperatively, it reached 0.7. There was no complication during the surgery and at postoperative follow-up.Öğe Treatment of Xanthelasma Palpebrarum By Secondary Healing After Excision(Galenos Yayincilik, 2007) Akcali, Cenk; Saygili, Oguzhan; Inaloz, Serhat; Oksuz, Huseyin; Tamer, CengaverObjective: Xanthelasma palpebrarum (XP) are soft or hard yellow lipomatous plaques that are commonly located on the upper or lower eyelids and in the medial canthus. Various methods have been used in the treatment of XP. In this study, our aim was to investigate the efficacy of secondary healing after excision in the treatment of XP. Method: Thirteen patients with 29 lesions were enrolled on this study. After excision, the secondary healing of the lesions was investigated during a period of minimum two months minimum and 18 months maximum. Results: All lesions were removed with excision. After treatment, crusting was seen in 4-5 days and sloughing was observed within 6-8 days. The postoperative erythema disappeared in 14-24 days. Of the 13 patients, two (% 15,38) developed a recurrence within a mean follow-up period of ten months. In three patients (% 23,07) temporary complications were seen. The complications observed included minimal bleeding, local bacterial infection and minimal hypopigmentation. During follow-up, no patient had any complaints and they were all satisfied with the results. Conclusion: Because of the efficacy, cost-effectiveness and ambulatory applications of secondary healing after excision in the management of XP, this method is an appropriate choice of treatment.Öğe Vacuum-Assisted Continuous Circular Capsulorhexis Using Bimanual Irrigation and Aspiration System of Phaco Machine in Immature Cataract(Hindawi Ltd, 2013) Oksuz, Huseyin; Daglioglu, Mutlu C.; Coskun, Mesut; Ilhan, Ozgur; Tuzcu, Esra Ayhan; Ilhan, Nilufer; Ayintap, EmreSeventy-eight eye of 65 patients were enrolled in this retrospective clinical study. Two-side ports are made with a 23-gauge stiletto knife. The irrigation handpiece is introduced into the anterior chamber through one side port and 27-gauge cystotome is introduced through the other one. Anterior capsular flap is created with cystotome. The capsular flap is vacuumed with a 25-gauge visco elastic's cannula, which connected to the phaco machine vacuum. The continuous circular capsulorhexis (CCC) is completed with the using bimanual irrigation and aspiration system of phaco machine. Vacuum-assisted CCC technique was used in 78 cases of uncomplicated immature senile cataracts. All cases were done under sub-Tenon's anesthesia. A complete CCC was achieved in all cases. Performing CCC with our technique is easy, safe, and cheap. It may be an alternative method to CCC by using OVD and forceps.Öğe Vitreous Incarceration After Ranibizumab Injection: An Ultrasound Biomicroscopy Study(Slack Inc, 2015) Helvaci, Sezer; Sahinoglu-Keskek, Nedime; Kiziloglu, Mustafa; Oksuz, Huseyin; Cevher, SelimBACKGROUND AND OBJECTIVE: The aim of the study was to evaluate vitreous incarceration at the injection site using ultrasound biomicroscopy (UBM) after intravitreal injection of ranibizumab. PATIENTS AND METHODS: The study included 39 eyes of 34 patients who underwent intravitreal injection of 0.05 mL of ranibizumab. UBM (HiScan; Optikon, Rome, Italy) with a 50 MHz probe was performed 1 day after surgery to determine the existence of vitreous incarceration at the injection site. RESULTS: The mean patient age was 59.7 (+/- 10.1) years, and 58.9% were female. Vitreous incarceration into the pars plana site was detected in six eyes (15.3%) the day after intravitreal injection. CONCLUSION: Vitreous incarceration at the injection site after intravitreal injection is thought to lead to a higher risk for complications such as infection, retinal breaks, retinal detachment, vitreous hemorrhage, and fibrovascular proliferation at the injection site. To understand the clinical importance of vitreous incarceration, further long-term prospective studies are necessary.
